Guiding Principles • Continuation of optimal quality care to HIV/AIDS patients during and post transition • Existence of a Faith Based Constituency recognized by the Government of Kenya • Creating a winning formula that guarantees continued funding to the LPTFs • Capacity building approach to transition through a consultative process

Transition Approach • Identification of local partner (s) primarily within the faith based sector. • MEDS Prime • CHAK & KEC Subs for site management, HSS and aspects of M&E • UON – Lead Clinical sub-grantee • Capacity building of the Local partner led consortium • Induction; Mentorship/shadowing; Training • Periodic review based on performance benchmarks • Exchange learning experiences • International Partners Support but with exit strategy • Incremental transfer of functions, responsibilities and resources to the local partner led consortium • Transfer of full management responsibility of 4 LPTFs to the local consortium • Monitoring transition and capacity transfer • Performance benchmarks • Updating transition plan • Re-strategizing and repositioning

Preparations for Post Transition • Defining roles and responsibilities for each partner • Developing capacity building plans for the MEDS led consortium • Incorporating the capacity building plans in the budgets • Monitoring implementation of capacity building plans • Preparing the MEDS led consortium to respond and win an RFA

Challenges • Rapidly changing transition landscape • Concerns over possible opening of the RFAs to both local and international NGOs and how this affects LPs competitiveness • Managing the need to build LP capacity with existing staff and the sustainability of the current consortium members in the absence of a transition budget • Incomplete discussions on a possible 4th year of transition

Overcoming the Challenges • Rapidly changing transition landscape • Scenario building & continuous engagement of all partners • Focus on what needs to be done • Concerns over possible opening of the RFAs to both local and international NGOs and its impact LPs competitiveness • Creating a strong but versatile consortium that can respond to any eventuality
