  1. New Jersey Cross Part Collaborative Statewide Data for Ryan White funded HIV Programs Pamela Gorman, RN, ACRN and Terri Fox, MSW

NJ Cross Part Collaborative History • October 2008 the National Quality Center (NQC) funded to facilitate the collection of Statewide HIV AIDS Bureau (HAB) performance measurement data across all Ryan White funding streams • 18 month training program for implementation • All participating agencies volunteered to commit to the program

  3. Statewide HAB Performance Measurement Selection • HRSA/HAB Performance Measurements were nationally vetted prior to dissemination • NJ CPC Statewide team selected the initial performance measurement project - annual syphilis screening • Currently the performance measurements are selected by NJ CPC with input from participants attending the NJ All Grantee Meeting

In+Care Campaign • NJCPC joined the In+Care Campaign, a nation wide initiative to track performance measures on patient retention • Measures were developed by a workgroup of nationally renowned researchers and providers • In+Care Campaign participants report this data to the state and directly to the In+Care Campaign website • Participation grants access to resources and best practices from all parts of the United States
