USMLE Step 3or… Oh yeah, I keep meaning to do that… • Take in Second Year or Early Third Year • http://www.fsmb.org/m_usmlestep3.html • Cost: $690 • Online app: http://www.fsmb.org/usmle_applonline.html • Need: • Computer with printer • Credit card • e-mail address • Certification of Identity (Notarized)

Senior Talk • Required for graduation • Opportunity to share with colleagues your scholarly activity • One-hour lectures given by each member of the graduating class to fellow residents and faculty during the noon conference time. • Topics include: • Quality Improvement projects • Presentation of studies in which you have been involved during your residency • Discussion of your research • Description of an advocacy project that grew from your Outreach experience • Focus of your special or away month into an educational lecture • Literature review with a didactic session about a case or topic in which you are interested • Each senior presentation should be prepared in PowerPoint format. • Topics should be submitted to the Chief Residents by January 9th.

State Licenseor… can’t I just keep my training license? • www.ncmedboard.org • Online application that you download • General Licensing Application • 25 page application • Takes “4-6 months” of processing during busy months • Will need your File ID #, same as renewing training license (there is a way to look it up on website) • Cost: $ 388 -Hidden Costs (Fingerprinting, score reports etc.) Approx: $150

NC State License • Will Need: • 2 Letters of Reference • From physicians, they just have to fill out the form • Ask for these early as no processing occurs until these are in • 10 year letter can be very brief, just need to verify you are not a sociopath • Certification from residency program • You can get this from Kelly Lear • Certified photograph (go to Sears cheap and professional) • Official verfication from med school that you were a student there • You’ll have to attach a picture to the form and then send it to your med school; they will sign it and send it back to you; then you send it to the medical board • Simple, right? • USMLE Scores (www.fsmb.org - $50.00 per recipient) • AMA physician profile ($31.00 or free if AMA member) • 2 sets of fingerprints • may be obtained for $10.00 at public safety office (2-3951; Joyce Beeson; M-F 9:00-11:00 and 1:30-4:00)

NC State License • You have two options for your full license. • Using FCVS and not using FCVS • We recommend not using FCVS • FCVS is an extra step (OPTIONAL) • CONS: • Time: may hold up application, rec. starting in Oct • cost $295+ • PROS: Only helpful if you plan on moving from state to state • It is a central repository for documents so would save you from collecting every time you apply for license • CHECK your application status a couple weeks after all documents submitted • They will not alert you of missing items • Some items you thought you turned in may not have been received

National Provider IDor… that one’s news to me! • New number to track physicians with a national database, in order to prevent fraud and track malpractice • Required for all HIPAA covered physicians • http://www.cms.hhs.gov/NationalProvIdentStand • You already have one, but the copy of the confirmation may be needed by multiple sources so figure out where that is now. • You can get another letter if you need by going to https://nppes.cms.hhs.gov/NPPES/Welcome.do • The reports can be requested from the • National Practitioner Databanks web site www.npdb-hipdb.com.

Practitioner Report Nonsense • National Practitioner Databanks web site www.npdb-hipdb.com. • Required piece for NC license. • Fill out form, have it notarized. • Will receive report in mail and then should include in NC license application. • Pay close attention to which copy you send in and which copy you keep

Hospital Accreditationor…maybe all outpatient sounds good… • Each hospital has their own packet • Will likely require proof of insurance, available from Legal • 966-3041, ask for Angela • Will likely need immunization/TB records • Can get from Occupational Health (6-4480) • Cost: ~$ 350 • Usually covered by your practice • Don’t underestimate the time and paperwork for this part. Keep copies of all of your licensing prep work as many of the same forms are needed.

Peds Boardsor… maybe I need to start studying… • www.abp.org • Register under ‘General Pediatrics Certification’ • Monday Oct 12, 2009 • Requirements: • Completed 3 years residency • Verification by Program Director • Cost: • Reg registration: Dec 2-Feb 26 ($1760) • Late registration: Feb 27-May 7 ($2060) • To study, don’t forget about www.pedialink.org (everyone has an AAP # needed to sign on)

  13. Bottom Line: Cost Estimateor… have you seen my bank account? Estimated Total Cost: $3000-$3300 Tax Breaks: Can deduct as professional expenses. Best to do all in one tax year as deduction only starts after a minimum threshold is met. (Disclaimer: I am not an accountant)

Get Started soon… • You will need multiple copies of your med school and master's diplomas, marriage license (if you have had a name change), Social security card, and passport for various things. • Make 10-15 copies of all of these • May want to scan them into a PDF file. • Consider making a 3-ring binder with marked file folders containing various copies • Photos: Many license/credentialing applications and Step III require Sears will do 2 poses and several shots for around $20 (sheet of wallets). • Walgreens, phot shop, target, etc • Start a CV and begin to update it. • University of Michigan has nice guidelines • The guidelines can be found at http://www.med.umich.edu/medschool/faculty/.   Click on CV Guidelines in the right column.  • Start a file with all relevant addresses. • Keep addresses of your med school, residency program, hospitals that you have worked in after residency • It may also be useful to keep the name and addreses of folks that write your letters of recommendation • Simple forms that verify you were a resident here can be filled out by Kelly or Danielle on Dr. Hamrick's behalf. • If you need an actual recommendation letter please contact Dr. Hamrick. • Keep track of dates and forms that are still needed as many of these agencies are not flexible with deadlines or with required documentation. Please get started as soon as possible because if you are not licensed or credentialed you cannot work and you will probably need the money come the end of the year!
