  3. On a multi vessel operation in the GoM, an FRB was used to perform a personnel transfer to the chase boat – After the transfer the FRB was behaving very sluggish & slow. There were no errors indicated on the dashboard. The jet was cleared but that didn't help & the bucket movement seemed fine. The Bridge of the Mother vessels was informed of the situation & the Source vessel’s FRB which was in the water as the stand-by boat was informed & asked to come over to offer assistance if required. After stopping the engine the crew could hear that the bilge pump was running but upon opening the engine compartment it was found to be completely flooded. The Source vessel’s FRB then came alongside & took Mother vessels FRB in tow and the chase boat also came beside as a stand-by. Incident Review • Incident Description

  4. Findings Incident Review • Summary of findings and root causes • Zinc anode on the oil cooler had unscrewed and dropped into bilge. See picture. • Faulty high bilge water level indicator • Improper maintenance. • “As Built" Electrical Drawings from manufacturer not received

Main actions & lessons learned Faulty high bilge water level indicator replaced Maintenance checklist updated Revised work instructions call for in water test on painter line after any maintenance Crew instructed in proper use of manual bilge pump Shared with Small Boat Manager and other vessels via Knowledge database.
