  1. Submitter’s Toolbox Present Prepare Collect Assess Submit • Team Leader follows step-by-step instructions on how to use and create a new NQF Adoption Survey. • All information is put into the online survey tool, including answers to the questions and comments about supporting documentation or activities. • Online submission is completed and saved, and survey is printed for future reviews. • Dashboards provide overview of results by campus with option to drill-down to Safe Practices detail. • The deadline is still to be determined for first submission and, unless otherwise noted, the most recently submitted survey will be used for analysis. • Quality team presents overview of Portal and process for submission. • Quality Leaders follow up with campus team and clarifies any issues. • Campus Team Leader to obtain a hard copy or PDF version of the NQF Safe Practices for Better Healthcare: A Consensus Report. • Download and print a hard copy of the NQF Adoption Survey from the Campus Programs section. • Pull together a team of “internal experts” who can address the following sections: • Improving Patient Safety by Creating and Sustaining a Culture of Safety • Improving Patient Safety Through Informed Consent, Life-Sustaining Treatment, Disclosure, and Care of the Caregiver • Improving Patient Safety by Matching Healthcare Needs with Service Delivery Capability • Improving Patient Safety by Facilitating Information Transfer and Clear Communication • Improving Patient Safety Through Medication Management • Improving Patient Safety Through Prevention of Healthcare-Associated infections • Improving Patient Safety Through Condition- and Site-Specific Practices • Assign team member responsibilities for individual survey sections (mapped to above sections). • Develop an NQF Adoption Plan to complete by (date to be determined). • Each team member collects and identifies documentation (as needed) to support survey answers. • Inventory of source documents for each Safe Practice can be organized and posted to the CareLeaders workbench for future reference and survey submissions. • Use NQF Adoption Survey to clarify any issues that relate to the answers. • When more information is needed, please contact: • Dr. James Pappas JPappas@llu.edu • 909-558-9798 • Virginia Mullen VMullen@llu.edu • 909-558-4637 • Perform review with all team members to determine if other supporting documentation or activities might have been overlooked. • As needed, review answers to test inter-rater reliability with survey answers among team members. • Assemble consolidated paper survey using answers from each team member. • Collect comments regarding answers to the survey. • Team members and Leader perform final review before online submission and Campus Administrator sign –off.
