Technical • Targeting services and improving effectiveness by use of risk stratification: • Use an NHS tool to stratify population at risk of hospital admission • PARR (out-of-date hospital data) • Other real time (MIQUEST interrogated) • Plot populations and individuals • Compare with LA data

  5. High risk >65: PCT scoring ASHH clients >65

  6. So….. • How do health and social care data map? • Gaps may indicate unmet need - who doesn’t each statutory services know about? • In the highest risk group, 80% were actually admitted within one year as predicted • Need to establish cost-effectiveness of intervention strategies

  7. Social • Domestic violence police • Call-outs by MOSAIC group • Data linkage • Individual level knowledge • Developing multiple risk profiles • Alcohol • Safeguarding • Developing intervention strategies

Economic • Local teaching hospital group set up a company to source local food • Net saving £3 million with further £3 million back into the local rural economy • Saved 150,000 lorry miles annually • Linked to local food group and supported roll-out to all LIFT buildings locally • Potential for £400 million of savings nationally a year

  10. Making it work • Get the basics right • Agree what ‘Joint’ means • Who leads on health in the LA • Is health in your Community plan • Are LAA targets aligned • Do you really know what you are aiming for.. • Share staff/information/advocacy/citizen engagement
