  6. M/V KALAKALA • What happened: • Vessel listed 20° to port • Water intruded into the bilge • 100 gallons of motor oil in the engine • Significance: • Threat of sinking or capsizing • No power to the bilge pumps • OSLTF opened

M/V HYUNDAI OAKLAND • What happened: • Hazmat release of Parachlorobenzotrifluoride from a container • 2 people received chemical burns • Cause was unsatisfactory blocking and bracing • Significance: • Release of hazardous material • Personnel injured • Unsatisfactory blocking and bracing found in 6 containers

  8. P/C ANDORINA • What happened: • Vessel grounded and sank as the tide came in • Vessel started to discharge • OSLTF opened for dive ops and fuel removal • Significance: • Submerged for duration of response • 150 gal potential • Owner financially unable to pay for response • OSLTF opened

F/V RUBY LILY • What happened: • Vessel grounded • OSLTF opened, but insurance agency took responsibility • Vessel had 30° list during defueling, ops halted • Vessel refloated with the tide • Significance: • 4800 gallon potential spill on a State Park • 4 contractors hired with 5 vessels on scene • Ballast concerns during defueling
