  1. Identification, Driver’s License, and Social Security Cards

  2. The FLASHCard is your Kent State University Campus ID • Use your card for • Library • Rec Center & athletic events • Food plan • Debit account.  • In order to get your Kent State University FLASHcard you must come to room 104 in the Kent Student Center with a photo ID.

  3. STATE ID CARD • Must provide proof of name, date of birth and social security number (if you have one). • You may apply for an Ohio Identification Card at any local Bureau of Motor Vehicles (BMV) office. The fee is $8.50. • Acceptable Documents (one primary and one secondary.)

  4. Driver’s License • Procedures vary based on country of origin • Written and or road test may be required • Temporary license may need issued first • Best source of information is the Digest of Motor Vehicle Laws, and Kent or Ravenna BMV office • Fees for a Driver’s License vary depending on age. • 18 – $21.75 • 19 – $20.50 • 20 – $19.25 • 21 or older – $23.00

Secondary Primary HEALTH INSURANCE CARD Example: Blue Cross/Blue Shield STUDENT IDENTIFICATION CARD - With photograph issued by school or University FOREIGN BIRTH CERTIFICATE – Must be accompanied by a translation by an approved translator MARRIAGE CERTIFICATE OR LICENSE - Must be original CREDIT CARD US CITIZENSHIP AND IMMIGRATION SERVICES(USCIS)- Your original I-20 or DS-2019 (no copies). VALID PASSPORT SOCIAL SECURITY – if you have one. Proof of Residency - Bank Statement, Bill, or Lease

What Should I Bring to The Social Security Administration Office? • I-20 • Passport • Kent State University ID • Admission Letter to Kent State University • Social Security Employment Verification Form (SSEV) which you will only receive after you get a job (on campus) and come to the Office of International Affairs.
