This report details the implementation of the Power Play! program supplemented with HOTM in two school sites. The intervention involved 200 matched surveys and 12 weekly lessons for 4th and 5th graders. Teachers were trained uniformly to deliver nutrition education, culminating in improved student knowledge and participation. Family newsletters were an integral component of the program. Lessons included PA activities and a grocery store field trip.

Using this example, let’s fill out a final report together… Example: • California County Local Health Department, new in FFY 2013 • Intervention was a combination of Power Play! supplemented by HOTM with 4th and 5th graders in 6 total classrooms in 2 school sites • 200 matched surveys were collected • Intervention group was a convenience sample; there was no comparison group • Teachers were all trained together and each teacher implemented the intervention identically; classroom teaches delivered all the nut ed • Surveys were self-administered with classroom-level guidance • Lessons were taught 1x per week for 12 weeks; each lesson was 1 hour • Students had 10 PP! SIRK lessons, used the PP! workbooks, and used 2 PP! collaterals • Students brought home HOTM family newsletters, used the menu slicks, and each month featured a produce item that was taste tested • Each lesson had a 10 min PA activity at the end; there was one field trip to a grocery store; 2 lessons were based on Dairy Council materials
