Overall Flow • Data entered remotely via SAFE on the SHC portal/intranet • Managers alerted by email of event added to their SAFE-MIDAS+ Worklist • Trained managers: • Review and enter additional event information • Make and respond to referrals • Provide feedback to reporters • Patient Safety develops reports

  3. Click on the SAFE app

  4. SAFE Welcome Page Click here

  5. Event Reporting Form for Physicians

  6. After you click Send • “Thank you for your submission” message displays for Physician user • Final step: • “Click here to exit this window”, or • “Click here to report another Event” • Event or suggestion details emailed to Patient Safety • Event details entered into SAFE system • Assigned to Manager for follow-up • If you provided contact information, Patient Safety follows up with status of review.   7. Wrap-up • Training for SAFE event reporting for all users occurring now in February • Switch over from PSN to SAFE on March 1st • Trend Reporting to start in late March • Reports presented to PPEC, QIPSE, QSC and MSU Committees
