关于 2012-2013 学年初高中学校评价数据采集工作的通知. 此项工作依据北京教科院基教所指示，由区中教科、教科所部署的一项工作。调查问卷分为教师、学生、家长、干部四种。 1 、学生和家长问卷由各年级的二班完成，请班主任将家长问卷放学前发给学生，带回去由家长填写，明天收回。 1 、教师问卷已经发给各位年级组长，请老师到年级组领取，请于周五（ 14 日下午 4 ： 30 ）前，由年级组长收齐问卷，交至教科室。 2 、初、高中跨头的老师分别填写初高中两份问卷，分别交至您所授课的初、高中年级组长处。

  1. 关于2012-2013学年初高中学校评价数据采集工作的通知关于2012-2013学年初高中学校评价数据采集工作的通知 此项工作依据北京教科院基教所指示，由区中教科、教科所部署的一项工作。调查问卷分为教师、学生、家长、干部四种。 1、学生和家长问卷由各年级的二班完成，请班主任将家长问卷放学前发给学生，带回去由家长填写，明天收回。 1、教师问卷已经发给各位年级组长，请老师到年级组领取，请于周五（14日下午4：30）前，由年级组长收齐问卷，交至教科室。 2、初、高中跨头的老师分别填写初高中两份问卷，分别交至您所授课的初、高中年级组长处。 3、每位干部填写两份问卷（初高中干部调查表），任课的干部填写三份问卷（到所在年级领取问卷一份）。 感谢各位年级组长、所有二班的班主任及各位教师的支持！！！
