  3. BRANDING • Branding completed at all CHCs but not started at all sub centers and PHC Palodra, Bedla & Losing out of the visited institutes.

  4. CLEANLINESS Toilets are blocked & required repairing at CHC TD & CHC badgaon . c Tender for Cleanliness is done at all 3 CHCs, pending at CHC TD( tender floated in the month of Sep. 13. Tender for Cleanliness done at 2PHC (Barapal & Chawand), pending at 3 PHC . Activity not started at sub centers. CMHO & DPM informed. Entrance of CHC Badgaon is very unclean .

  5. MMU/MMV • 3 MMU & 9 MMV are allotted to Udaipur district, all vehicles are functional. • 158 camps were organized by MMU in which 7727 patients were treated till Sep. 13. • 959 camps were organized by MMV in which 45269 patients were treated till Sep. 13 • Stickering completed.

  6. WIFS Blue tab of IFA is available at all visited schools. WIFS program is operational at visited schools except one (jktdh; ek/;fed fo|ky; dfM;k). Blue tab of IFA is available at all visited AWCs. WIFS program is operational

  7. ASHA • 3 ASHAs were interviewed. All 3 were not trained in ASHA module 6/7. They are following children discharged from MTC & FBNC and are participating in sector meeting & VHSC meetings. But not distributing DOTS medicines. • PAYMENT:- ASHAs are receiving payments for :- • Monthly meeting • VHSC meeting • On follow-up of children discharged from FBNC/MTC • ASHA drug kit is not provided to ASHAs.

  8. JSY GRIEVANCE REDRESSAL BOX:- • Grievance redressal box is not available at all visited health facilities. JSY PAYMENT:- • JSY payments to pregnant women are not pending at all visited health facilities.

CITIZENCHARTER ARSH • Citizen Charter is not displayed at any health facility which was visited. • ARSH clinics are not functional at visited health facilities except CHC Badgaon, PHC Bedla& PHC Palodra. SEVERLY ANEMIC PREGNANT WOMEN • SEVERLY ANEMIC PREGNANT WOMEN are not being tracked at any visited health facility. SHUBHLAXMI YOJNA • Payments of Shubhlaxmiyojna is not pending at all visited health facilities

  11. 108 Ambulance • 27 vehicles are allotted • Out of which 22 vehicles are on road and 5 vehicles (JhadoleSarada, Gingla, Kalibheeth, Kherwada and Bekariya )are off road due to under repairing. • Staff Available :- 2 driver, 2 ENT and 1 reliever • Stickering not done, Tender called in the month of Sep. 2013
