FY 2013 CACFP Application • Late August an email will be sent to the Authorized Representative regarding FY2013 contract • MAKE SURE YOUR CONTRACT IS CURRENT WITH THE CORRECT AUTHORIZED REPRESENTATIVE’S CONTACT INFORMATION • Contract Due Date: September 17 • Download 2013 Application Manual • http://www.dpi.wi.gov/fns/cacfpapps.html • FY2013 contract is not available until the email is sent • Until then you are accessing FY2012 contract

FY 2013 CACFP Application Changes • General Information Page: • Adds a box for theExecutive Director’s information; if applicable Executive Director is defined as the Chief Executive Officer or managing director of an organization, company, or corporation. The role of the Executive Director is to design, develop and implement strategic plans for the organization. The Executive Director is also responsible for the day-to-day operation of the organization, including managing committees and staff and developing business plans in collaboration with the board for the future of the organization. The Executive Director is accountable to the Chairman of the Board and reports to the board on a regular basis.

  6. FY 2013 CACFP Application Changes • Estimated Enrollment Information • Clarifies that reported estimated monthly enrollment is for ALL sites participating in the CACFP

FY 2013 CACFP Application Changes • Board-Official Relationship and Meetings Information • All Non-profit agencies and those for-profit agencies required to have a board of directors • For-profit agencies will be asked “Is your for-profit organization required to have a governing board?” If yes, you will be required to complete information on board members

  8. Board of Directors – Additional Board Members and Financial Interest Questions 1 A majority (>50%) of board members cannot have a financial interest in agency 2

Transfer of Information in Contract from “Old” to “New” Questions Information previously entered into first two boxes of FY2012 contract on Board-Official Relationship and Meeting Information page will be pre-populated into first two boxes of FY2013 contract; information from third box will be deleted. Check what is in boxes and change to correct responses because chances are the information in the box will not pertain to the new question asked.

FY 2013 CACFP Application Changes • Board members cannot be family related to any agency personnel performing CACFP duties as listed on Staffing Personnel page of contract • Board members cannot be family related to each other • Executive Director or Authorized Representative cannot be family related to a board member • Family related members are defined as an individual’s spouse, domestic partner or similar designation, and the individual’s or spouse’s (or domestic partner’s) children, grandchildren, great grandchildren, siblings (whether by whole or half blood), and the spouses/partners of the children, grandchildren, great grandchildren and siblings

FY 2013 CACFP Application Changes • The Executive Director or Authorized Representative can be a member of board provided a resolution is in place • Resolution - Bylaws, articles of incorporation or board polices and procedures include a resolution that the Executive Director or Authorized Representative is not eligible to vote on items related to the board’s decision regarding their salary or other human resource issues that affect them, such as hiring and firing.

  14. Board of Directors – Board Members Employed by Agency Question • Enter the number of board members employed by agency • Agency is allowed to have one member of board be employed by agency • Board member cannot be listed in application as performing any CACFP duties

FY 2013 CACFP Application Changes • Publicly Funded Program Information • Only list NEW publicly funded programs the agency or principals have participated in during the past year • Do not delete existing records • Examples of publicly funded programs include W2 Shares, CACFP, Head Start "Publicly-funded program" means any program funded whole or in part by federal, state or local government. "Principal" means any individual who holds a CACFP related management or supervisory position within, or is an officer of, an institution or a sponsored center, including the executive director, all members of the institution's governing board of directors or similar body, or a sponsored center's governing board of directors or similar body.

FY 2013 CACFP Application Changes • Controls C Page (Sponsoring Organizations) • Clarifies the question about monitoring policy (Monitoring sites 3 times/year is required per GM5) • Detail your policies and/or procedures for supervising agency staff that monitor sites participating in the CACFP. These policies and/or procedures must ensure effective management and monitoring of all CACFP activities at all sites • How will your agency assure sites receive monitoring visits and within the required time frame? • Who will oversee staff completing monitoring visits? • Who will assure that any follow-up is completed?

FY 2013 CACFP Application Changes • Additional information you may need to send DPI • Updated license obtained since last fall • Vendor Agreement for Fiscal Year 2013 • Any updates related to PI-6070 (Sponsoring Organizations) • Health and Safety Standards documentation (At-Risk, OSHCC, or Emergency Shelters)

Amending Your CACFP Contract Notify DPI immediately when any of the following changes occur throughout the year... • License changes • For site location, days and hours of operation, and license capacity • Fax or send revised licenses immediately upon receiving them from DCF • Staff Changes • Authorized Representative of the contract • Staff responsible for CACFP duties • Contact Information • Address, email, phone • When changing contact information do so on General Page, not on page requesting contract preparer’s information

  21. LOGOUT!!! • When renewing contract or making changes • Click on "I agree and accept the certification" and "Submit" buttons • Click Log-out before you close out of the screen • Email your Consultant
