Important Start Dates 2019 - Fall All Physicals will be due to the Athletic Office on the following dates: Cheerleading and Football - On or before Aug 7, 2019 Soccer, Field Hockey, Cross Country & Girls Tennis - Aug 7, 2019 Tryouts for Fall Sports are as follows: Football and Cheerleading - Aug 12 All other fall sports will start on Aug 12

  37. Parent Permission Form Please complete the entire form and Provide best number for you to be reached at the bottom and date.

  38. History Form When filling out the history form, please do not pick and choose the items to complete. Fill out the entire form!

  39. Athletic Injuries • All athletic injuries must be reported to the coach and Athletic Trainer ASAP. • Secondary Insurance paperwork will be complete and mailed home. • Parents must submit Secondary Insurance forms to AIG within 90 days of injury. • Athlete must seek treatment within 30 days of the injury. **You must follow the rules of your primary insurance before using the Secondary Insurance Policy.

  40. Concussions • A brain injury that can be caused by a blow to the head or body that disrupts normal function of the brain. • Can range from severe to minor. • CDC estimates 300,000 concussions per year. • Most concussions do not involve loss of consciousness. • Time to recover varies with each injury. • Returning to play with symptoms leaves the athlete vulnerable to second impact syndrome.

  41. Concussion Referral to Full Participation • Referral to you Primary Physician. • Return forms with a diagnosis to the Athletic Trainer. • Daily symptoms records will be taken. • Once asymptomatic, complete Post-Injury ImPact test. Must return to baseline. • Complete one week of school without symptoms. • Complete a graduated 5-step return to play following the RTP guidelines. There is a 24 hour period between steps and if symptoms return the athlete returns to the previous step. • Athletic Trainer will get final clearance from the school doctor to allow the athlete back to full participation.

  42. ImPact Testing • Baseline testing is a necessary component for concussion care. • Allows for comparison of athlete to themselves at time of injury. • Testing dates are scheduled prior to the beginning of each season. • Tests are valid for 2 years. • If the test is deemed invalid, the athlete will retest immediately.

  44. Medication Forms All medications that are needed in school have to be signed by parent and doctor. • Any medication that the student can take himself such as an asthma inhaler also has to fill a pupil self medication form in addition to the medication form. • All over the counter medications also need to have a medication form signed by parent and doctor. • Students are not allowed to carry medication on their person with exception of an epipen, diabetic medications and asthma inhalers.
