8. Target performance for KPI 2 for 2008/09 and future years Hertfordshire County Council www.hertsdirect.org For 2008/09, an overall target for Hertfordshire of 65% is consistent with the following differentiated target levels, according to client group of the service: Night shelter services reportapproximately half of all the departures recorded for single homeless services, and currently show a much lower level of planned outcomes. Services for single homeless primary client group will need to be the main focus for work towards achieving the Hertfordshire county wide target.

This will be possible by creating custom reports from the SPOCC It system but this breakdown of the data is not available immediately. Recommendation To adopt targets for Local Area Agreement NI 141 as presented in Table 4.

Barriers and Solutions to Move On Hertfordshire County Council www.hertsdirect.org Issues to consider Barrier: Poorly planned move on leading to repeat homelessness (setting people up to fail) and the pressure to move people on Solutions: • Homelessness providers should ensure that procedures are in place and that staff have the relevant expertise to make move on decisions at the right time, in consultation with the client. Staff should accept that move on may be hard for clients. • Clients should be offered education around issues that aid successful move on (budgeting, cooking, shopping) as discussed above, perhaps involving ex-service users in peer support. • The risk assessment and reference process (detailed above) should ensure that relevant support is available for clients as they make the transition to other supported accommodation or their own tenancies. • Move on targets should be used to ensure no silt-up.

11. Strategy Hertfordshire County Council www.hertsdirect.org Barrier: Lack of strategic planning around housing, support and care services. Solutions: • LA’s should take the lead on strategic planning, involving all agencies (statutory and voluntary) in the area using a local partnership approach. • Local Area Agreements (LAA’s) should be used to further develop homelessness services and take a wider more strategic approach to homelessness.

Solutions: • The strategic planning and local partnership approach, outlined above, should encourage partnerships between all agencies and providers (statutory and non-statutory) at all levels. • Planning clients’ integrated housing, support and care needs in partnership with other services/agencies should start from time of initial assessment. Joint assessments with all relevant agencies would be beneficial. • Joint commissioning of relevant housing, support and care services should occur where this would be beneficial. • Effective communication between different agencies providing housing, support and care to an individual should be ensured.

13. Actions Hertfordshire County Council www.hertsdirect.org District Authority • Nominate a representative from each district authority to set up local move-on project group, alongside Supporting People representative, voluntary sector representative and where possible service user, to look at homeless services in their area. Supporting People • Co-ordinate the distribution of the MOPP analysis toolkit to homeless service providers per district, in partnership with the district project groups • Co-ordinate project meetings with district partners

14. Steering group Hertfordshire County Council www.hertsdirect.org • A steering group is to be set up to monitor the progress of the district project groups and the findings of the Move-on analysis. The members of the steering group will be: • Chair – Mark Janes – Lead officer • A representative from the Supporting People Team – Louise Gotch • Provider representatives-one from a direct access hostel, one from a short term accommodation based service - TBA • Representatives from each district where possible, Probation, PCT’s , voluntary sector - TBA

15. Project groups Hertfordshire County Council www.hertsdirect.org Will seek to: • Map services, access and referral routes both into, and on to other permanent provision • Identify , level of need, type of provision and geographical spread for move-on accommodation • Identify key blockages and suggest options for improving the through-put of clients to PDG. • Include consideration of the role of district allocation policies, and incorporating move-on targets into homeless strategies action plans • Evaluate possible models of good practice, provision and mechanisms for ensuring effective working arrangements between stakeholders. This includes identifying any further work required to enable the model to work effectively, such as supported housing panels, service level agreements, protocols or procedures.

16. Timescales Hertfordshire County Council www.hertsdirect.org It is intended that the findings of the initial MOPP analysis, completed by all providers of single homeless services will be reported back to PDG in July and September Commissioning Body.
