  3. Development • In July 2012 hospital CEOs and hospital leadership requested a report that showed the extent of their hospital’s participation in the program • The first version of this report was sent to implementation team members and CEOs July 2012

  4. Version 2.0 • Many of you gave us feedback and we have incorporated it into the report • We have modified some of the wording • Added additional program activities

  5. New Categories • Formally designated a physician champion • Used CEO Checklist • In-person meetings

  6. Formally Designated A Physician Champion • Many of you already have physician champions helping to lead this project • Send the name of your physician champion to us • Help us create a network of physician champions across the state

  7. Used CEO Checklist • This document should be used by the CEO or their designee • Your CEO/designee should ask one of your surgical teams to show them how they use your hospital’s checklist • This tool should not be used with a live patient • This tool should be used after you implement the surgical checklist

  8. In-Person Meetings • For this phase of the work we are planning to hold a total of 3 in person meetings, like today • The first meeting was held in October • The next meeting will be held April 17that the Patient Safety Symposium

  9. When You Receive Your Report Today. . . • Review it carefully • Tell us who your physician champion is • Contact us if we do not have the correct information • We need all of your updates by February 7th so we can include the correct information on the version that we will send your CEO
