What are we doing? • PIP’s – Performance Improvement Teams • Last Staff meeting – introduced teams. • Did Education about the PSDA process – PLAN, DO, STUDY, ACT • Bulletin Boards have been placed in every office – a place for you to keep up with PIP activities and changes that have been made.

  4. What are our 3 PIP’s? • #1 - Increasing MVHPC staff presence at the time of Death – beginning with examining patient wishes surrounding death.

  9. § 418.112 Condition of participation: Hospices that provide hospice care to residents of a SNF/NF or ICF/MR Biggest change in this COP . . . An agreement that it is the hospice’s responsibility to provide services at the same level and to the same extent as those services would be provided if the resident were in his or her own home.
