OBJECTIVE: • Provide operations personnel with an understanding of the various chem-bio agents and their effects. • Utilize victim symptoms to assist in identifying chemical agents. • Provide operations personnel with a systematic approach to rescue actions at chemical incidents. • Refresher training for decontamination procedures • Practical exercise in decontamination

BIOLOGICAL Living – Bacteria and Viruses • Living organisms that reproduce • May or may not be contagious • Plague, Small Pox & ANTHRAX Non-living – Toxins • Poisonous byproducts of microorganisms. • Not living or contagious • Venom, Botulinum (neurotoxin –fast acting) • Ricin (cytotxin – slower acting)

BIO Response Procedures • Credible vs Non credible • Isolate Area and Establish a Perimeter. • Quarantine persons in immediate are of exposure. • Recover and double bag substance, package if unopened. • Shut down all air handling systems. • Use the term Bio Agent over radio, not specific agent.

  5. Bio Response Actions If product is suspect, deemed credible and package is opened follow hazmat procedures for isolation, product packaging and decontamination of victims.

Packaging the Product • Opened vs Unopened • Collect the following info for health agency. -Incident time, date, location. -Names, address, contact info for potential exposed individuals. -Facility owner or manager contact info. -Credible threat details. -Investigating officer, I.C. or HazMat officer contact information. -Contact local health district or ESCA to coordinate testing by state lab. Police will transport packaged.

  9. RIOT CONTROL AGENTS • What are the signs? • Coughing, tearing and mucous production • How can you tell if a person has been exposed to a riot agent vs nerve agent. • No miosis – pinpoint pupils • No emesis – vomiting • No convulsions • No other SLUDGE?

  15. PPA PERSONAL PROTECTIVE ACTIONS • All responders in proper PPE • Park as listed in Operating Guidelines • Instruct victims to move from hazard area • Gross decon • Set up decon area • Run off not a concern • Notify local hospitals

  16. EMERGENCY DECONCONSCIOUS/AMBULATORY • Have patient remove clothing/jewelry *Bag & Tag • Brush dry powders • Protect modesty • Flush thoroughly • Dry and re-dress • Triage, Treatment & Transport

EMERGENCY DECONUNCONSCIOUS OR NONAMBULATORY • Remove all clothing/jewelry - Look for signs • Bag & Tag – ID patient • Wash contaminated areas with chosen solution • Rinse all areas • Cover with blanket and move to aid unit • Protect transport unit and personnel • Leave backboards in decon • Check Vitals.(are you cleaning a corpse)

  18. MASS DECON • Goal- Provide most benefit to largest number of victims in shortest possible time. • OPS responsibility (use of engine) • Contaminated? Tokyo Subway • Exposed? Mariner High School • Communicate responders intent

  19. COMMUNICATE RESPONDERS INTENT • SAMPLE COMMUNICATION (via P.A. or bullhorn) • Attention. You have been contaminated, Approach the Fire Engine. • This chemical must be rinsed off you body. Enter the water to rinse the chemical from your body. (Repeat) Stand with your arms extended outward and enter the water. • This chemical must be rinsed from your body. Allow your self to be flushed with water. (Repeat) • Listen carefully, proceed between the fire enginse for further rinsing. Go Now. (Repeat) • Strip your outer clothing off. Strip only your outer clothing off. Strip only your outer clothing off. (Repeat) • Please put on the white coveralls provided and receive medical attention. (Repeat)

Contaminated vs Exposed • Contaminated • Make impure, polute. • During a hazardous materials release the product is on your skin, clothing, etc. Your have direct contact with the product. • Exposed • During a hazardous materials release the product is in your vicinity. You did not come in direct contact with the product. You may have noticed and odor but not adverse affects from the release.

  21. MASS DECON PROCEDURES • Arrival – PPA • Determine # of Patients(a few vs. numerous) • Communicate with Patients • Set up mass decon corridor • Control flow of patients

  22. MASS DECONCONTINUED • Separate Male and Female • Give victims instructions on decon • Protect victim’s modesty • Dry and re-dress • Triage, Treatment & Transport

  23. KNOW YOUR ROLE Review your departments guidelines Make sure you have the proper equipment Protect yourself – PPE, recognition and planning Practice what you plan – you may need it! Review annually
