  1. CLAIMFORM – COMPLETIONGUIDELINES AND PROCESS • Your claim for Study Leave should be completed in accordance with the attached guidelines. • In order to guarantee payment, please submit the claim form promptly, within 1 month of the date of the course. Late claims may not be agreed for payment, particularly at year end in March. • It is important to follow the instructions carefully, as our documents are entered onto our payment ledger by optical character reader and if any of the information is unclear or missing, it will fail scanning and your claim will be returned to you unpaid. • Please attach all receipts and evidence of attendance and ensure that you and your Course Leader / Tutor has signed the form. • Once complete it should be returned to Anita Powell in the PMDE GP Team, at the address on the form. • It will be checked by the PMDE GP Team and will be approved for payment. • The invoice and supporting documentation will be scanned and passed for payment by electronic transfer. • If you have provided an email address, you will be sent a remittance advice to confirm that payment has been made. • If you have any queries about this process, please contact Anita Powell on 0121 213 2191 or by email: • anita.powell@westmidlands.nhs.uk

Please ensure that letters are not connected or the optical scanner will be unable to read and your claim will be rejected. Please leave these fields blank. They will be completed by staff in the GP Education Team. Please complete details of your course as directed. This should be supported by your attendance sheet and receipts. Further breakdowns of costs may be given on separate sheets if required. Please ensure that the your GMC number is completed. Please provide bank details in order that payment may be made electronically. Please enter the total of your claim here. Please do not cross out or alter this field as this will fail to scan. Please sign and date your claim. Please leave this field blank. It will be signed by staff in the GP Education Team once your claim is approved. The Course leader or Trainer should sign to confirm your attendance on the course.
