  7. Changes to the 2009 PATH ReportFull Reporting Beginning in 2009 PATH Providers will no longer need to calculate the federal percentage

  8. Changes to the 2009 PATH ReportReporting Estimates Providers encouraged to report on actualnumber of persons served, not estimates If providing an estimate, provide rationale for how estimate was calculated in text box in the B Table

  15. RemindersTable C, Services Provided For all questions in table C, indicate: • Funding source • Number of persons who received the service if the service was 100% or partially PATH funded • Enter zero (0) if the service was provided but not PATH funded or not provided

RemindersTable D, Demographic Information Demographic information reported only for consumers who became enrolled – • each question must add up to B3 (enrolled PATH clients) Exception is question D8 reported only for literally homeless – • each question must add up to D7a (living outdoors)+D7b (living in short term shelter)
