  5. Don’t stay silent! • Use your voice • to make BHB better • for patients, • visitors, family members • and staff.

  6. Share follow-up information from safety events and feedback to make improvements in your area and the organization as a whole.

  7. Did you know that feedback events • include compliments, suggestions, complaints • and inquiries?

  8. Unhealthy behaviour undermines patient safety and will not be tolerated. Disruptive events are for reporting unsafe and inappropriate behaviour.

  9. Compliments are important too! Remember to share compliments, cards, and acts of appreciation from patients and family members in Feedback.

  10. Top three safety events for 2011-2012: • Medication events • Laboratory events • Fall events

  11. Help the Quality & Risk Management team improve processes • for optimal • patient safety!

  12. Disruptive events are CONFIDENTIAL and only go to three BHB staff members -- remember your part in keeping information confidential.

  13. Be the change you want to see. Report actual and/or near miss events in Quantros – it can save a life!

  14. Act on recommendations that come • from safety events to prevent the same from happening again.

  15. Behind every event, there is a solution. • We value your input!

  16. Quantros provides a voice for staff • to act as advocates for patients, families and themselves.

  17. You identify the problem. Quantros allows for easier recording, categorizing and tracking of safety events.

  18. Whatis a safety event? • Any event or incident that is out • of the ordinary • or not consistent with an intended outcome.

  19. Who should report? Everyone! It is OUR duty to report. • Learning from safety events protects our patients and employees from harm and from professional liability.

  20. Why report? • To avoid potential harm to patients, employees & visitors through the identification of common high risk safety processes.

  21. When used correctly, the benefit of Quantros is safer care, which inspires confidence and supports our vision to be first choice.

  22. Submitting a safety event is only the beginning. Process improvement and education strategies start small and • gain momentum.

  23. Quantros data is used to guide BHB’s resource planning to ensure optimal clinical, financial and operational decisions that support quality and patient safety goals.

  24. Injured on the job? • All staff injuries should be entered as employee safety events.

  25. Quantros helps us to identify causes or contributing factors to systems failures and are used as the basis for improvements.
