Etiology

Fistulous communication between lymphatics and urinary tract

Secondary to lymphatic obstruction

Infectious: Filariasis (most common worldwide), TB

Non-infectious: Neoplasm, Pregnancy or surgery

Multiple case reports of chyluria after partial nephrectomy ( Kim & Joudi, 2009)

Possibly >2% of all partial nephrectomy patients (Miller et al, 2007)

Increased risk if complicated surgery with disruption of collecting system

Usually not present immediately post-op, but develops weeks later

Presentation

Frankly lipid-rich (i.e. “milky”) urine

Immune deficiency (secondary to Ig loss)

Recurrent UTI

Obstruction (secondary to ureteral clot/debris)

Or incidental finding on CT or urinalysis of no clinical significance

Clinical Course & Treatment

Reported post surgical cases are variable
