  1. Health and Social Care Integration in Kent James Lampert Families and Social Care Kent County Council Kent Adult Social Care Conference 2012: Shaping Care in Kent 21st June 2012

  2. Transforming Adult Social Care in Kent Our goal is: “That people are at the heart of all adult social care activities, receiving integrated services that are easy to access, of good quality and that maximise their ability to live independently and safely in their community.”

  3. By working in new and innovative ways with our partners in the NHS and organisations in the care sector, we will: • Focus on prevention and targeted interventions • Ensure that services respond rapidly and are more effective • Support carers and empower individuals to do more for themselves

  5. By 2034 there will be 2.5 times as may people aged 85+ (5% of population) • Age is a factor in prevalence of LTCs and multiple LTCs
