  5. Ethical Testing Acknowledgment Form • Form must be printed and signed by classroom teacher and assessment proctor • Attach completed form to score report • Teacher should retain in files • Will be required for technical assistance visits beginning in 2013-2014

ROSTER ISSUES • BEFORE SUBMITTING A TROUBLE TICKET • If a student does not appear on your roster, check with counselor or registrar to make sure student has been officially registered for the class • If student appears on roster and should be dropped, check with counselor or registrar to make sure the student has been officially dropped from class.

Trouble Ticket Process • Contact Assessment Office by email • assessment@arkansas.gov • Detailed message containing the following • Teacher Name • School Name • Course Name and Number • Student Name • Detailed description of the problem • Student not on roster • Student needs to be deleted from roster • Student needs assessment • ?????

ASSESSMENT Policies • When should a student NOT take the CTE EOC assessment? • ELL Levels 1 or 2 • A student that has ever completed portfolio assessment for any end of course academic assessment • A student that transfers into a semester class after the 1st nine weeks and has not been enrolled in the same class at his/her previous school • A student that transfers into a year long class after semesterand has not been enrolled in the same class at his/her previous school

  10. Documentation for students not tested • Print two copies of rosters – use one to distribute student logins and passwords, keep one as a record of enrollment • Complete documentation form (on website) for those students not tested • Attach rosters and documentation form to score reports • Keep all assessment documentation in your files for technical assistance visits
