  2. Case เด็กหญิงไทย อายุ 8 ปี 10 เดือน ภูมิลำเนา ต. ท่าเสา อ. เมือง จ. อุตรดิตถ์ ประวัติได้จากมารดาของผู้ป่วย ผู้ป่วย และเวชระเบียน ประเมินแล้วเชื่อถือได้มาก CC : ไข้สูง 3 วันก่อนมาโรงพยาบาล

PI : - 3 days PTA มีไข้สูงลอย ร่วมกับมีไอเสมหะ น้ำมูกใส ปวดศีรษะ ปวดเมื่อยตามร่างกาย ไม่มีคลื่นไส้อาเจียน ไม่มีถ่ายเหลว ไม่มีปัสสาวะแสบขัด - 2 days PTA (fever day 2) มา รพ ตรวจเลือด CBC : WBC 10800 , PMN 84 ,L 12.8, plt 330000 , Hct 39.5 ได้ยาลดไข้ แก้ไอกลับไปทาน ยังคงมีไข้สูง ปวดศีรษะ ไอเสมหะ น้ำมูก คลื่นไส้อาเจียนเป็นน้ำลาย 2-3 ครั้ง ทานอาหารได้น้อยลง ปวดท้อง ซึมลง ไม่มีจุดเลือดออก ไม่มีเลือดออกผิดปกติ - วันนี้มาพบแพทย์ตามนัด ยังมีไข้อยู่ CBC : WBC 8000 , PMN 83 ,L 12 plt 155000 , Hct 38 จึงให้นอน รพ

  4. Past history - ประวัติการฝากครรภ์และการคลอด • มารดาฝากครรภ์ที่ รพ ผลตรวจปกติตลอด • Serology negative all • คลอดปกติ ครบกำหนด BW 3100 gm • หลังคลอดแข็งแรงดี กลับบ้านพร้อมมารดา • ประวัติการเจ็บป่วยในอดีต • no underlying disease • no drug and food allergy • ปฏิเสธประวัติการผ่าตัดในอดีต

  5. Past history - ประวัติวัคซีนและพัฒนาการ • รับวัคซีนครบตามเกณฑ์ • เรียนชั้น ป. 2 • ประวัติประจำเดือน • ยังไม่มีประจำเดือน • ประวัติครอบครัว • ปฏิเสธการเจ็บป่วยเป็นไข้ของคนในครอบครัวช่วงนี้ • มารดาป่วยเป็นมะเร็งเต้านม

  6. Past history • ประวัติอื่นๆ • มีประวัติเดินทางไปจังหวัดเลยเมื่อ 2 สัปดาห์ ก่อนหน้านี้ (ในเมือง) • ปฏิเสธประวัติเข้าป่า ลุยน้ำ • มีคนแถวบ้านป่วยเป็นไข้เลือดออก

Physical examination BT 38.8 ,PR 120 full, RR 26, BP 100/60 BW= 23 kg, Ht= 131 cm GA : a Thai girl, good consciousness, not pale, no jaundice, no dyspnea HEENT : not pale conjunctiva, anicteric sclera, mild injected pharynx , tonsil not enlarge, no exudate , no oral ulcer, no cervical LN enlarge Heart: normal s1s2, no murmur, regular rhythm, tachycardia

Physical examination Lung : clear, normal breath sound Abdomen : no distend, normoactive BS , soft , mild tender at RUQ, no guarding, no rebound tenderness, liver and spleen cannot be palpated, splenic dullness negative CVA : not tender Extremities : no rash, no eschar, no petechiae Tourniquet test : negative

  9. Problem list Acute high grade fever with non specific symptoms

  10. Differential diagnosis • Non- specific viral infection • Dengue fever / Dengue hemorrhagic fever • Influenza • Viral gastritis • UTI

  12. Provisional diagnosis Dengue fever/Dengue hemorrhagic fever
