  1. Business Plan 2013 Our mission is to provide hope and surgical care to the world’s forgotten patients. We train medical professionals abroad in complex reconstructive surgery in order to create a sustainable infrastructure where long-term relationships are forged and permanent change is made.

  2. Executive Summary • Mission:Restore, a non profit organization established in 2010, delivers state of the art reconstructive surgical care to underserved patients in emerging nations. Of these tens of millions of forgotten patients, only a small fraction receive some form of treatment with either minimal or no follow up care. • Mission:Restore’s goals are to: • Provide expert reconstructive surgical care to patients globally • Educate and train medical professionals dedicated to volunteer care • Invest in scientific clinical research to improve plastic surgery practices and procedures • Our model is unique because each program is designed with an understanding of the culture, the country’s healthcare delivery system and the prevailing medical needs of each country. We believe in operating with the local surgeons not for them. By engaging local experts in the solution, the program is embraced and institutionalized. • We believe that a global blueprint will emerge for self replicating programs that are sustainable and accountable, each within a 5 year time frame.

  3. Executive Summarycontinued • We have established collaborative agreements in Afghanistan, Myanmar and El Salvador to begin programs during 2012 and 2013. These countries were identified because of the enormity of need and the lack of trained surgeons. • Mission:Restore is seeking $147,000 over the next calendar year to establish programs in: • Myanmar ---- $102,000 • Population: 55 Million • Total number of trained reconstructive surgeons: 5 • No existing certification program currently available • Established partnership with Yangon General hospital to create a 3 year certification training program • Haiti-- $45,000 • Population: 10 million • Total number of trained reconstructive surgeons: 0 • No existing certification program currently available • Plan to collaborate with Partners in Health to create 3 year training program • In order to proceed with the growth of programs in the Middle East, Central America and Africa, we are seeking an incremental $700,000 in 2014-2015 • With adequate funding we will achieve our goals to: • Establish a strong network of support in each country • Institute an effective training/education program • Formulate an effective monitoring system to ensure program execution and validated outcomes • Provide a sustained continuum of care and follow up for patients • Train seed surgeons who will be tasked with training the next generation of surgeons in their home country

  4. Vision Mission: Restore will be the world leader in providing highly complex reconstructive surgical training to indigenous surgeons in emerging nations. The surgeons complete certification programs that match the medical standards set in the United States and then train the next generation of surgeons thereby creating self sustainable infrastructures.

  5. Experience • Mission:Restore was founded by a group of American Surgeons affiliated with Long Island Plastic Surgical Group (LIPSG) • Established in 1948, LIPSG is the largest and longest continuously-running practice of its kind in North America , treating over 500,000 patients • LIPSG provided the seed money and administrative support to start Mission:Restore and provides volunteer surgeons and nurses to the organization. • Mission:Restore volunteer surgeons have collectively performed over 1500 complex reconstructive surgeries since 1993 in over 20 countries and 4 continents • Mission:Restore tackles complex cases such as car bomb injuries or acid burns on victims of war and trauma in order to bring new research to these reconstructive challenges. • In 1954, LIPSG established its own plastic surgery residency program. The LIPSG Residency Program has trained more than 125 residents. Many of the doctors have achieved national recognition. • As part of LIPSG’s residency program, Mission:Restore provides free care on a routine basis for underserved patients • LIPSG’s offices serve as the flagship for oversight of the educational programs of Mission:Restore

  6. Mission:Restore’s Expert Reconstructive Surgical Training Team LAURENCE T. GLICKMAN, MD, MSC, FRCS (C), FACS KAVEH ALIZADEH MD, MSC, FACS Dr. Kaveh Alizadeh is a partner at Long Island Plastic Surgical Group and Chairman of the Board of Mission: Restore. He obtained his undergraduate degree from Cornell University, his Masters Degree from Columbia College of Physicians and Surgeons and his MD from Cornell University Medical College. Dr. Laurence T. Glickman is a partner at Long Island Plastic Surgical Group. He received his MD degree from McGill University in Montreal, Canada and completed his General Surgical training at McGill University’s Montreal General Hospital. JOSHUA ZUCKERMAN, MD Dr. Joshua Zuckerman joined Long Island Plastic Surgical Group in 2011. He obtained his Bachelor’s degree from Amherst College, his Master of Science in Applied Physiology and MD from the Chicago Medical School, graduating first in his class and earning the Board of Trustees Award for Scholarship. Vincent R. DiGregorio, MD, FACS Dr. Vincent DiGregorio is the senior surgeon and partner at Long Island Plastic Surgical Group. He received his MD from Albany Medical College in Albany, New York. Dr. DiGregorio is Chief of Plastic Surgery at Winthrop University Hospital. LOUIS H. RIINA, MD Dr. Louis Riina is a partner at Long Island Plastic Surgical Group. He received his undergraduate degree from the University of Delaware and obtained his MD from the Sackler School of Medicine in Tel-Aviv, Israel. Dr. Riina is involved in the training of Plastic Surgery residents at Nassau University Medical Center. He is also a member of faculty at the Nassau University Medical Center Burn Center. .

Program Overview • Mission:Restore identifies countries where there are large numbers of patients in need of complex reconstructive surgical care • Mission: Restore travels to locations where local surgeons are committed to their communities • Mission:Restore partners with the local hospitals willing to house and support the surgical procedures and training programs • Mission: Restore builds sustainable training programs for reconstructive surgeons by: • Providing professional incentives for local surgeons to stay engaged in their communities • Utilizing American standards for completion of training programs leading to certification in conjunction with local ministries of health • Using telemedicine between surgical training missions to educate and share information with doctors in targeted locations. Mission: Restore volunteer surgeons conduct weekly Skype sessions with our partners. The use of technology enables our team to assist doctors with complex cases in between official Mission: Restore trips. • Optimizing information technology to record and report outcomes based on various educational formats • Mission: Restore shares Information with local surgeons to drive outcomes-based research on: • Evidence based medicine including treatment of complex burn contractures • Research into complex problems including use of skin substitutes in facial acid burns

Program Overview (long term goals) • Mission:Restorewill ultimately create: • Local chapters dedicated to issues specific to the country • An advisory board for each country with its own special panel of experts with dedicated information, expertise and fundraising ability • Country specific boards composed of experts with deep, cultural knowledge • Local chapters linked via internet to Mission: Restore’s U.S. home base to exchange information and knowledge • Institutionalization of country specific programs that will provide sustained care • Mission: Restore will serve in an advocacy role by: • Partnering with organizations to identify and promote awareness on the root causes of injury and effect change • Raise public awareness about the devastating effects of the root causes of the injury including explosive devices in war torn countries

  10. Organization and Management • Mission: Restore is a 501(c)3 not-for-profit corporation, a Type B corporation within the definition of the New York Not-For-Profit Corporation Law. The activities and affairs of the Corporation are conducted and all corporate powers are exercised by and under the direction of the Board. • Mission: Restore is governed by a Board of Directors comprised of leaders in the healthcare, technology, financial and nonprofit sectors (see appendix for member information). • The Advisory Board of Mission: Restore is comprised of highly qualified professionals with experience in business development, management, joint ventures, media branding, and philanthropy (see appendix for member information). • The Young Professional Board is composed of more than 15 young professionals ranging in age from 22-30 who are committed to support the mission through fundraising events, social media and marketing (see appendix for member information).

Organization and Management KAVEH ALIZADEH MD, MSC, FACS Founder and Chairman Dr. Alizadeh is a board certified plastic surgeon and has been a partner at the Long Island Plastic Surgical Group since 2000, where he also served as President from 2007-2012. Dedicated to advancing the field of plastic surgery, Dr. Alizadeh directs the Clinical Research Division of the practice’s Residency Program. Dr. Alizadeh obtained his undergraduate degree from Cornell University and earned his Masters Degree from Columbia College of Physicians and Surgeons. Dr. Alizadeh's passion is to perform volunteer plastic surgery. His volunteer work began in the Afghan refugee camps in 1993 and since then he participates in volunteer missions each year traveling all over the world including Central and South America, the Middle East, and Asia.  In 2009, Dr. Alizadeh’s humanitarian volunteer work earned him the prestigious Ellis Island Medal of Honor, an award recorded in the United States Congressional Record. In 2010, Dr. Alizadeh helped found Mission: Restore, Long Island Plastic Surgical Group’s foundation which provides medical care, education, and research to help adults and children in need around the world.  BERNADETTE WALSH Executive Director Bernadette leads the daily operations of the Foundation including managing fundraising strategies, corporate/private donor relations, media campaigns and events, as well as grant proposals and budget enforcement. Bernadette brings over twenty years of experience in corporate communications, public affairs, nonprofit management and fundraising. Previously, she was the Executive Director of the Avenue of the Americas Association, a non-profit civic improvement corporation made up of Fortune 500 companies in New York City. Bernadette also worked at New York City’s Economic Development Corporation, where she oversaw all public relations and marketing activities of this not-for-profit corporation. Prior to EDC, she worked at the global public relations firm Burson Marstellar, where she managed several global accounts including the U.S. Treasury and U.S. Army Reserve, which entailed filming medical missions to South America and Europe. She also holds a master’s degree in in public policy and public administration from Columbia University’s School of International and Public Affairs. MATTHEW R. REDER Controller Matthew leads the daily financial operations of the Foundation, including financial statement reporting, liaising with external accounting firm, and budget development. Matthew is also the Controller of Long Island Plastic Surgical Group since 2011, where he is responsible for the day-to-day financial operations of the medical practice. Most recently, Matthew served as Senior Director of Financial Reporting for Catholic Health Services of Long Island. Matthew also spent six years in senior roles in two healthcare-focused finance companies, after beginning his professional career with five year tenure at Big Four accounting firm KPMG. Matthew graduated Cum Laude from Loyola College of Maryland (now Loyola University), Sellinger School of Business, with a Bachelor’s Degree in Accounting.

Kaveh Alizadeh MD, MSc, FACS, ChairDr. Alizadeh, a board-certified plastic surgeon, has been a partner at Long Island Plastic Surgical Group since 2000. He served as President at LIPSG from 2007-20012. Dr. Alizadeh directs the Clinical Research Division of the practice’s Residency Program. Kathleen Reichert, Vice Chair Kathleen Reichert is Executive Vice President of American Skin Association. Kathleen has been involved in the healthcare field for over 20years. Fred Nazem Fred Nazem has been an entrepreneur, venture capitalist and corporate turnaround specialist for more than 25 years. Mr. Nazem has started, financed and guided more than 100 companies, including six that have become multibillion-dollar enterprises. In 1997, as chairman of Oxford Health Plans, he led the reorganization and successful reversal of the company. Arman Falsafi, Treasurer Arman Falsafi is a seasoned executive in the technology and financial sectors, most recently as as Managing Director of Global Electronic Trading & Data as well as General Manager for Europe, Middle East & Africa at CME Group, the world’s leading derivatives marketplace. Advisory Board: Experienced professionals in law, finance, health care and marketing. Marketing Strategy • Position Mission:Restore as a leading global not-for-profit organization that not only provides reconstructive surgical care for the world’s forgotten patientsbut further educates and trains local medical professionals in host countries to help establish and deliver long term sustainable care. • Our messaging in the US would focus on the broader corporate mission and will be designed to help secure partnerships with the medical community, teaching institutions, pharmaceutical companies and other key constituents; • Within local host countries, Mission:Restore’s efforts will focus on building local buy-in and ownership to effectively secure high profile local ambassadors/spokespeople, donors, influencers and medical institutions/facilities to help build and sustain ongoing initiatives. • Motivate key stakeholders such as high profile donors, ambassadors and spokespeople with compelling patient stories while also focusing on the scientific metrics, effective training and ongoing monitoring programs to engage and compel the medical community, academia and pharmaceutical companies and ultimately drive them to action. • Identify key themes, collect video footage and capture compelling images to help bring the stories to life; • Provide data points, scientific metrics and long term impact to validate Mission:Restore’s efforts. • Tactically, we will leverage: • High profile personalities to raise visibility and amplify Mission: Restore’s message; • Media – in the form of publicity (free media) and advertisement (paid media) – to tell our story; • Online and digital platforms to further engage the audience with news & activities and increase fundraising (missionrestore.org, Facebook and twitter) ; • Speaking opportunities to establish thought leadership; • Awards and Ranking opportunities to raise Mission: Restore’s profile; • Grassroots efforts to drive membership and amplify Mission:Restore’s voice in the community.

Appendix Mission: Restore Young Professional Board: Mission: Restore’s Young Professional Board (YPB) is a group of emerging leaders dedicated to raising community awareness about the organization, as well as fundraising to support the organization’s medical and surgical training missions. The goal is for the YPB to grow with Mission: Restore, helping to build the financial and volunteer support network necessary for the organization to thrive.

Appendix Mission: Restore Medical Student Chapters: The goal of the Mission: Restore Medical Student Chapters is to raise awareness about Mission: Restore amongst medical students so that they can become involved in volunteer medicine early in their careers. Unlike other nonprofit medical organizations that conduct much-needed medical surgical missions and then leave after a week, Mission: Restore has created a three year medical education and surgical program to achieve widespread, permanent impact. • In 2013, Mission: Restore is seeking $147,000 over the next calendar year to establish programs in Myanmar and Haiti. In order to proceed with the growth of programs in the Middle East, Central America and Africa, we are seeking an incremental $700,000 in 2014-2015. • We believe that a global blueprint will emerge for self replicating programs that are sustainable and accountable, each within a 5 year time frame. By the end of a ten-year period, Mission: Restore will have trained over 100 surgeons and changed the lives of over 100,000 patients by the end of a ten year period.
