Kendrick Road Bridge General Scope of Work • Widening Kendrick Road Bridge Structure Widen Abutments, Pier, Pile Foundations, Embankments, Approach Slabs and Pavement Widen Bridge Deck, New Sidewalk, Lighting, Utilities, Railing Mill Top 2 Inches and Concrete Overlay Existing Deck • Provide Access Ramp to I-390 NB New Ramp, Pile Foundations, Piers and Superstructure Frame into Widened Steel Structure Replace Safety Shape and Noise Wall Acceleration Lane Pavement, Shoulder • Traffic Signal at Kendrick Rd./Ramp Intersection • Repair/Modify Canal Wall • Highway and Street Lighting • Provide WZTC

Kendrick Road Bridge General Information • Lump Sum Design-Build Contract • Two Step Design-Build Procurement Process • Project Schedule • Total Duration 18 Months +/- • There will be a Payment Schedule provided in the RFP • Stipend Under Consideration by NYSDOT • Liquidated Damages may be imposed for missed milestones and final completion • All Environmental Permits and ROW by NYSDOT • Construction Related Permits by Design-Builder

Kendrick Road Bridge Quality Control (QC) • QC is Design-Builder’s Responsibility • Design-Builder shall appoint a QC Manager responsible for QC Plan for both Design and Construction • Design QC shall be by the Designer in accordance with the QC Plan • Construction Inspection shall be by an Independent Engineering Firm (Hired by the Design-Builder) • Materials Testing shall be by an Independent Testing Firm or Laboratory (Hired by the Design-Builder) • The Independent Testing Firm shall report to the Construction Inspection Firm who shall report directly to the Department • Offsite Plant Quality Control will be performed by the Department

  18. Kendrick Road Bridge Quality Assurance (QA) • QA is Department’s Responsibility • The Department may perform QA on any of the Design-Builder’s activities • The Department may utilize an independent Consultant to perform QA of the Design-Builder’s Designs
