NY Route 347 – Design Build General Scope of Work • Widening of NY Route 347 with additional travel lanes and shared-use path for pedestrians and bicyclists in a Manner that Maintains the Corridor Theme; • Pedestrian facility design and installation; • Bus turnouts and enhanced bus stop design and installation; • Extensive landscape design and installation including, but not limited to: vegetated screening, planted raised median, street trees, vegetated drainage features; • Sound wall design and installation;

NY Route 347 – Design BuildAdjacent Contract PIN/Description:0054.08 (D261357) – NY 347 Safety and Mobility Improvements from 347/454 split to NY 111 • Remaining Work: With the exception of the construction of a recharge basin on the southeast side of NY 111 (behind McDonald’s) the work is substantially complete. The basin and “punch list” work is anticipated to be completed during the spring of 2013. • WZTC: Limited off peak daytime lane closures are anticipated. • Contractor: Grace Industries

NY Route 347 – Design BuildRight-of-Way Acquisition The Department anticipates that all additional ROW it deems necessary for the Project will be acquired prior to the start of work on the Project, (April 4, 2013) in accordance with DB §107-22. No relocations are required as a result of this project. Should the Design-Builder identify any additional ROW needs for the project, it shall be acquired in accordance with DB §107-22.

NY Route 347 – Design BuildUtility Status In accordance with Part 4 – Utility Requirements, Design-Builder is responsible for: • Verifying utility facility locations; • Identifying & resolving potential conflicts not yet documented; • Providing information on the need for additional utility easements (if required); • Coordinating with owner for utility relocation; • Provide stakeout and final layout for utility relocations • Protecting existing utilities; • Cost to repair utility damage caused by Design-Builder; • Informing the Department of all meetings/correspondence with utility owners; • Utility Agreement Process, including signing the Agreement

NY Route 347 – Design BuildUtility Status Long Island Power Authority (LIPA) and Verizon-Aerial • Approximately 26 poles carrying electric, telephone and cable on both sides of NY 347 (to remain in service) • Poles are owned by LIPA or Verizon • House connections to homeowners originate from the pole line along NY 347 (to remain in service) • There are third party attachments, including Cablevision and LI Fiber • Crown Castle has an antenna on pole # 249. • There are 4 radial feeds to the communities to the north and south of NY 347

NY Route 347 – Design BuildUtility Status National Grid Gas • There is an existing 8” steel main in the center median of NY 347 • National Grid to perform the relocation out of the center median • This relocation is anticipated to take 6 weeks • There is an existing 8” steel main along Mt. Pleasant Rd (to remain)

NY Route 347 – Design BuildUtility Status Verizon(U/G) • There are 6- 4” PVC conduits (3 across and 2 deep). and a buried copper cable on the south side of NY 347 from NY 111 to Mt Pleasant Road • Conduits have approximately 30” cover and runs along the existing south curb line • There are 5 manholes and two pedestal boxes associated with the conduits. The two pedestal boxes are on the south side of NY 347 in the vicinity of the Town recharge basin • Verizon will need at least 10 weeks (each of the 5 Verizon manholes are fixed objects that cannot be relocated) to relocate their duct bank and buried cable • Verizon’s pedestal boxes will be relocated approximately 5’ north of their current location in order for the sound wall to be built • This relocation will take approximately 12 weeks • Construction work by owner’s subcontractor

NY Route 347 – Design BuildUtility Status Suffolk County Water • There are 12”, 8” and 6” water mains from NY Route 111 to Sequioa Ave. • These water mains may be relocated to the south side of NY 347 by the Design Builder • The Design Builder will be responsible to include in their bid price all associated costs to design and build these mains. This includes but is not limited to, connecting to the 12” distribution main on the north side of NY 111 by the Hess Station and all service connections, hydrants and valves • In addition there are 12” and 10” water mains in the vicinity of Mt. Pleasant Road. These mains may be impacted during construction. If they are impacted the cost of relocation or protection in place is to be included in the Design-Builder’s price for the project.

NY Route 347 – Design Build Contract Information • Lump Sum Design-Build Contract; • Best Value Selection Process; • Project Completion Date – April 30, 2015; • There are No Milestone Dates; • There is a Payment Schedule provided in the RFP; • $ 85,000.00 Stipend to Unsuccessful Proposers; • Liquidated Damages may be imposed for not meeting Final Completion date; • Escrow Submission of Bid Documents for Successful Proposer;

  22. NY Route 347 – Design Build Contract Information • There is a Buy America Clause • There is an Iran Divestment Clause • There is a DBE Participation Goal • All Environmental permits and ROW by NYSDOT; • Construction Related Permits (i.e., SPDES) by Design-Builder.

NY Route 347 – Design Build Quality Control (QC) • QC is Design-Builder’s Responsibility • Design-Builder shall appoint a QC Manager responsible for QC Plan for both Design and Construction • Design QC shall be by the Designer in accordance with the QC Plan • Construction Inspection shall be by an Independent Engineering Firm (Hired by the Design-Builder) • Materials Testing shall be by an Independent Testing Firm or Laboratory (Hired by the Design-Builder) • The Independent Testing Firm shall report to the Construction Inspection Firm who shall report directly to the Department • Offsite Plant Quality Control will be performed by the Department

  25. NY Route 347 – Design Build Quality Assurance (QA) • QA is Department’s Responsibility • The Department may perform QA on any of the Design-Builder’s activities • The Department will utilize an independent Consultant to perform QA of the Design-Builder’s Designs

NY Route 347 – Design BuildAdditional information – Submission Guidelines • All proposals must be self-contained. • Do not have links to external web sites. • Must contain all information needed for evaluation. • Do not “assume the Department knows” your firm and it’s experience/qualifications.

  28. NY Route 347 – Design BuildAdditional information – Submission Guidelines Submit ALL Forms required. Do not increase the size of the Form unless indicated on the Form. Most are set at a single page. Forms that can be extended: FPA – Part2. No appendices or attachments. Do not include photographs or illustrations unless specifically indicated. Do not submit Form SA (Stipend Amendment).

  32. NY Route 347 – Design Build Questions and Answers • What are the format and size requirements for the WZTC and Design drawings to be submitted as part of the proposal? Are 11”x 17” drawings acceptable? Answer: Yes, 11”x 17” • Are CADD files of the Preliminary drawings available and will NYSDOT provide to Proposers? Answer: Yes. • Are Inroad files containing horizontal and vertical information available and will NYSDOT provide to Proposers? Answer: Yes. • Preliminary drawings are in Metric. Do drawings prepared as required by the RFP need to be in English or Metric? Answer: English. • Do drawings prepared as required by the RFP need to follow NYSDOT CADD drafting standards? Answer: Yes.

  33. NY Route 347 – Design Build Questions and Answers continues… • Are we going to receive the InRoad files for the preliminary plans that were done by GPI, and if so, do you know when? Answer:Yes. The files should be available by the time the Final RFP is released. • Does the Department intend to provide the Proposers with digital files containing any of the info shown in the Indicative Plans??? Or are the proposers expected to create their own base mapping and preliminary design files for the project? Answer:Base mapping and preliminary design files related to indicative plans will be provided at the final RFP stage. • In the file Final Design Report /Final EIS/Final Section 4(F) Evaluation – Volume 3     is there a Part 1 to Appendix A – Typical Sections & Plans? It starts with part 2. • Along the same thought, is there a Volume  II – Appendix C – asbestos report-1   ?    There is a two and three.

What we would like to know is whether, during the RFP phase, the proposers will be permitted to collect additional data from the site? If so, would a Highway Work permit be required needed to perform data collection work within the state right-of-way? Answer: The Design-Build Teams will be permitted to gather additional information from the site. However, no lane closures will be permitted and a Highway Work Permit must be obtained from NYSDOT (for insurance purposes) prior to the start of any investigative work.
