  1. Welcome This module describes the process for initiating a CMA application for a Well Re-Entry. An Authorization Request may be required if the applicant is not the designated representative of the Crown agreement or the well licensee. To the ETS – Crown Mineral Activity Well Re-Entry Online Training Course

  3. Introduction • In this module, you will learn how to: • Complete Admin Tab • Complete Wells Details Tab • Submit the Well Re-entry Application Form

  4. Login to ETS 1. Login to ETS with your user name and password 2. Expand Crown Mineral Activity 3. Select Well Re-entry

  5. Well Re-Entry Tabs • Admin - this tab contains Contact information and Technical Contact, if needed. • Well Details - this tab displays the Well ID, Agreement, Formations, Terminating Location, and other Well Information for this CMA type.

  6. Complete Admin Tab • Enter Comments, • Select Company Name 2. Enter Contact Information Enter Technical Contact Information if available. 3. Click Save

  7. Complete Well Details Tab – Attach Letter of Explanation 2. Browse and select a PDF File. 1. Click on check box if required 3. Click on Save A Letter of Explanation is required when an existing approval is due to expire or has expired. Browse and select from your directory to attach this document. After saving, proceed with the rest of the Well Details tab.

  8. Complete Well Details Tab – Enter Well ID 4. Enter Unique Well Identifier

  9. Complete Well Details Tab – Add Agreement Number 1. Click on the find button ( ) to search for a Crown agreement 2. Enter Crown Agreement Number 3. Click on Search 4. Select the checkbox 5. Click OK Ensure to SAVE after completing a search for a Crown agreement.

  10. Complete Well Details Tab – Well Details 1. Enter Previous Approval Number if applicable 2. At least one Purposeand one Substance must be selected. An AER Order is mandatory if the purpose of Repair/Re-abandoned is selected. Browse and select a PDF file for the AER Order.

  11. Complete Well Details Tab - Formations 1. Click on the Formation dropdownto select an option 2. Click Add 3. Select and Add a TerminatingFormation from the dropdown 4. Click on Save The Terminating Formation must be selected if different from the (In the) Formation.

  12. Submit Well Re-Entry Application Click on Submit Click on the View Report link to review the details of this CMA Application.

  13. Submit Well Re-Entry Application - Indemnification Click on Yes Click on OK Refer to the Work In Progress module for more information on the status of a submitted Application Request.
