  1. Improvement Calculator, and Harm Across the Board Overview Presented by Jackie Conrad RN, BSN, MBA Improvement Advisor, Cynosure Health May 9, 2013

  2. Webinar Overview • Improvement Calculator • Harm Across the Board

  3. Connecting to our purpose…..

  4. Improvement Calculator

  5. Improvement Calculator • Calculates baseline rate • Pre populates targets and cost per HAC • Graphs monthly rates • Calculates YTD Improvement • # of lives impacted • Costs avoided • Gap to target

  6. Click on L4 Enter # of 30 day all cause readmissions for baseline period

  7. Click on M4 Enter # of discharges for the baseline period

  8. Both the Goal % and the Cost $ can be customized

  9. As monthly data is entered, the calculator will calculate your improvement

  10. Calculator will estimate the # of readmissions to prevent to reach the goal next month and graph your results.

  11. Harm Across the Board Calculation • Once data is entered for applicable topics, harm across the board will be calculated • Harm across the board sums all harms divided by patient discharges • ADE • Falls with Injury • Stage III/IV Pressure Ulcers • CAUTI • CLABSI • VAP • SSI • EED • VTE

  12. Discharges must be manually entered. Tip: Cut and paste from Readmission tab Harm Across the board includes all harms except Readmissions

  13. Live Demo

  14. Preparing the Harm Across the Board Template

  15. Harm Across the Board • Hospital templates are due the week of June 3 • HRET will select top stories to be presented at July 24-25 Quality Meeting • Participants will vote on top stories • Top stories will be on stage with Don Berwick at the meeting

  16. TEMPLATE GUIDE • Treat harms as events that can be summed • Focus on harms (outcomes) rather then preventive measures (process) • Special conditions can be considered a harm (e.g., EED, Readmits, …) • Produce an overall harm trend for the hospital (**Delete this slide when content of presentation is complete)

  17. 2012 Breakthrough in Reducing HARM: 250 to 50 harms/1,000 discharges Harm rate is per 1000 discharges Tip Cut and paste graph from Harm Across the Board tab in Improvement Calculator

  18. Cut “harm across the board” in half: 60 patients per quarter to under 30 Tip Cut and paste raw # graph from Harm Across the Board Tab in Improvement Calculator

  19. 2012 Breakthrough in Readmission: From 20% of discharges to 10% of discharges Tip Cut and paste graph from Readmission Tab in Improvement Calculator

  20. 2012 Breakthrough in Reducing Readmissions: From 20 per quarter to 10 per quarter Tip Cut and paste Readmission raw #s from Readmission Tab in Improvement Calculator into excel to create this graph.

  21. Pearls • Please list the drivers of safety that produced these results. • Include one about patient and family engagement, if relevant

  22. DefiningMoment(s) In Our Journey • Name and date one or two defining moments. • Moments that caused the organization to commit to extraordinary safety. • Moments that resulted in a big breakthrough in the organization’s ability to deliver safety.

  23. Strategies to Drive Results • What challenges did you encounter that you were able to overcome to achieve the results you are presenting here? • What were the strategies you used to overcome them?

  24. Risk Profile by Areas of Risk Tip Use Baseline Data. Indicate if sub-groups have been studied by editing text, i.e. ADE # of pts on Coumadin, SSI # of ortho surgeries, Readmit # of CHF inpatients. Customize to meet your needs.

  25. Improving Harms by HAC • Scale: number of hospital-acquired conditions (HACs) at each level • IDEAL: level represents what we see as best possible • At Target: level represents meeting improvement target • Progress: level not yet at target • Opportunity: level represents an improvement opportunity (**Delete this slide when content of presentation is complete)

  26. Improving HAC Rates (per discharge) Tip Pull baseline rates , targets and current performance from Improvement Calculator. Insert time period in brackets in column headers.

  27. Our Hospital Risk Profile & Result Tip For total risk #, use total from Risk Profile Slide

  28. Future Actions to Reduce Harm • What other actions will you take to reduce harm in the future?

  29. Photo of Hospital CEO &Safety Team

  30. Final HAB Template due week of June 3 • Breaking it down • Fill in Improvement Calculator by 5/14 • 5/14 HRET HAB Webinar if needed • Enter Readmission and Harm Across the Board data (4 slides) into HAB template by 5/17 • Enter Risk profile data by 5/24 • Complete Improving HAC rates by 5/31 • Get a picture of your CEO with your Team
