  2. Breakthrough in Reducing HAC HARM*: Rate decreased from 13 to 9.46 per 1,000 patient days. Goal Met *HAC harm = inpatient hospital acquired conditions

  3. 2012-2013 Breakthrough in Readmission*: Decreased rate from 14% to 11.6%. Goal Met *all cause 30 day readmissions

Pearls • Leadership must commit to making patient safety a top priority. • Safety Culture is the “underpinning” for all improvement work. • Senior Management buy-in is critical, but frontline staff need to be empowered to drive both patient safety initiatives and improvement work. • Frontline staff need to understand and be able speak up when they have a concern, and to use safety culture tools such as SBAR, huddles and debriefs.
