  2. GENERALITES • HTP= pression porte>12mmHg ou gradient pression porto cave>5mmHg • ETIOLOGIES: les blocs hépatiques • INTRA +++ : cirrhose • INFRA : thrombose porte (CHC surtout) • SUPRA = Budd chiari

  3. GENERALITES • CONSEQUENCES: • Splénomégalie, hypersplénisme • Circulation collatérale+++ • Dilatation veineuse d’amont • Varices oesogastriques • ascite • Quelles implications en chirurgie? • Colorectale • Voies biliaires • pariétale

  4. MORTALITE • Liée au degré d’IHC • Liée au geste chirurgical • Diminuée si conditionnement pré opératoire optimal: • Nutrition adaptée • Sevrage OH • Contrôle ascite • Augmentée si geste réalisé en urgence

  5. MORBIDITE • Hémorragies digestives • Hémorragies intra péritonéales • Infections • Ascite • IHC

CHIRURGIE COLO RECTALE • Indications: identiques à pop générale • Geste chirurgical: • Non modifié par l’HTP sauf proctectomie(1/3 non réalisable) • Morbidité : infections, hémorragies per op et post op, fistules • FDR de mortalité: ascite pré op, urgence Meunier Dis col rect 2008 Rapport AFC 1993

  7. CHIRURGIE COLO RECTALE • Coelioscopie: • CI relative • Contrôle hémostase difficile • Pb de l’ascite et de l’hépatomégalie • Stomie: • Difficile avec ascite • De protection: controversée • Drainage: pas de données précises Knoock, J Laparoendosc 2000 Meunier, dis col rect 2008

CHIRURGIE DES VOIES BILIAIRES • Cirrhose augmente le risque de lithiase biliaire • Cholécystectomie: • Conditionnement pré opératoire indispensable • Coelioscopie : • LC>OC : moins complications • contre indiquée pour cholécystite chez CHILD C (prothèse, cholécystostomie) WU JIWorld J Gastroenterology 2005 Pessaux, la lithiase chez le cirrhotique 2007

CHIRURGIE VOIES BILIAIRES • Modifications techniques de la LC: • Trocard ombilical latéralisé, sous xyphoïdien à droite • Trans-illumination (veines collatérales) • Traction vésiculaire modérée • Vérification orifices trocards • Drainage: compliqué par ascite Pessaux, la lithiase chez le cirrhotique 2007

  10. CHIRURGIE PARIETALE • HERNIE OMBILICALE : 20% des patients ascitiques • Ttt chir > ttt conservateur • Moins d’étranglement • Chirurgie réglée > chirurgie en urgence • Assèchement ascite pré op recommandé • Prothèse: moins de récidive, plus d’infections He A Marsman, Surgery, sept 2007

  12. CONCLUSION • HTP = FdR de morbidité de chirurgie digestive • Indication doit être formelle • Gestes opératoires peu modifiés sauf coelio • Importance conditionnement pré opératoire • Intérêt de TIPSS pré opératoire ?
