  3. Must start SAT procedures to complete file by due date. Gifted Assessment Schedule File due date no exceptions. Files must be complete and submitted to diag. center by these dates

  4. Initiate Student Assistant Team meeting • Parent or teacher formally requests a Student Assistant Team Meeting • Teacher requests a gifted nurse assessment from school nurse (G 2). School nurse will e-mail (G-2) to SAT chair within 10 working days • Teacher completes forms for gifted referral (G 1) • Teacher Provides AIMS Documents to SAT Chair before SAT Meeting - Sandra • Copy the large registration card (found in Debbie’s office) and put it in the folder

  5. How to find SAT forms • Go to the Intranet • Click on Department Websites • Click on Special Education Intranet Site • Click on Evaluation Services

  6. Form G-1 Cannot process SAT packet without current information Allow 10 working days to process. This form must be included in packet or packet will not be accepted

  7. G-1 thru G-6 complete a gifted referral for non PHLOTE students Most likely students will require a bilingual review if they are monolingual Spanish or participate in a bilingual program All packets must be submitted to diag center by the due date on assessment schedule

  8. NURSING ASSESSMENTG-2 • Teacher will request Nursing Assessment in the nursing department at your school. • Nurse will log in the request • Once the request is complete the nurse will e-mail the SAT chair G-2 form within 10 working days • Once the SAT chair receives G-2, they will print it andplace in the teacher’s mailbox

  9. Form G-3 Parent must receive parent invitation 10 days prior to SAT meeting Provide parent invitation in contact language Provide parent with current SAT contact information

Conduct Student Assistant Team Meeting SAT will only need to meet once SAT does not need to document Tier 1 and Tier 2 interventions SAT will provide necessary documentation on Gifted SAT Summary form (G-4) SAT obtains parent signature on Consent for Gifted Screening (G 5) SAT completes factors (G 6) worksheet with parent during SAT meeting SAT Chair will provide parent with the Parent and Child Rights during SAT meeting (email) SAT Chair will provide stamped addressed envelopes of all students being screened by Educational Diagnostician

  11. Form G-4 Add current assessments from AIMS report Check all in attendance during SAT meeting

  12. Form G-5 Parent permission should be obtained during SAT meeting Packet will be not be accepted and delayed without parent signature Provide parent a copy of this form with screening date.

  13. Form G-6 Refer to Language Usage Data form/Parent Interview

  15. PHLOTE Student • Complete Language Usage Data form for PHLOTE students: Phlote status can be found on back of registration card or AIMS Please note bilingual review • Most likely students will require a bilingual review if they are monolingual Spanish or participate in a bilingual program.

SAT Chair Submits Referral to Diagnostic Center • SAT Chair schedules a delivery date and time with Diagnostic Technician. • SAT Chair delivers Gifted SAT files attached to Gifted Log Sheet to Diagnostic Technician. • Diagnostic Technician provides a copy of the Gifted Check-In Log to the SAT Chair. • Diagnostic Technician notifies Educational Diagnostician of referral.

  17. GIFTED REFERRAL TRACKING LOG SAT chair must provide information below with complete SAT Packets attached to diag. center by due date. Do not leave diag. center without a copy of this form

  18. After the Diagnostician The Diagnostician will review the SAT file sent to him or her. A determination will be made as to whether this is a good referral or not. Additional request for information will be sent to be completed before further action can be taken. Upon completion of the additional forms and the referral is considered valid, the file now belongs to the Special Education Department.

  19. Diag. will inform parents of test results.
