  2. Washington State IIS • A statewide, lifetime immunization system that keeps track of immunization records for people of all ages • A secure web-based, time-saving tool for healthcare providers and schools

  3. Immunization Data Exchange Partners Private Pediatric Offices Medicaid Private Family Practice Offices Washington Health Plans Local Health Districts Tribal Health Centers Community Health Centers

  4. Who can Access the System? Organizations with licensed health care professionals (RN, LPN, MD, ARNP) on staff who need to know immunization status in order to provide or coordinate health care including: • Private and public health care practices • Public and private schools with school nurses on staff • Head Start, Early Head Start, and ECEAP programs with nurses on staff

  5. School Use of the System School nurses and other staff assigned by the school nurse can: • Look up a student’s immunization record • View the immunization forecast of vaccines due or past due • Print the Certificate of Immunization to give to the family to review, sign, and turn in to the school

  9. Enter username & password then click Login Note: Forgot Password Link

  10. More information about logging in • Do not let your computer remember your password • Do not share your password with anyone • Each user must have his/her own individual user account with a unique password • Passwords expire every 120 days

Enter User Name OR User Name and Email Then click on Send • Click “Forgot Password” on Login page to reach this screen • Enter only your User Name OR User Name AND Email, and click on Send button • A new password is emailed to you automatically as long as your correct email address is on file in the system • Or call our Help Desk at 1-800-325-5599 and we will reset your password

Hint: Never use your back button!!! #1 - Click on Search button after entering info in Patient Search #2 - Click here to select this record • Search by first name, last name, and DOB • Search by first and last initials and DOB • Search by first and last name, no DOB Navigate around the system by using the left hand menu or the clickable buttons on each screen

  13. Make sure you have selected the correct record by reviewing the information on this page.

  14. Note that red ‘x’ on date means invalid dose Click on Vaccinations then on View to bring up this page

  15. Reason why dose is marked as invalid Name of medical organization that entered the dose Click on Cancel to go back to previous page

  16. Report of disease for Varicella reported here • Contraindications include report of disease for varicella • Forecast reflects ACIP recommendations – not always same as school requirements

  18. Step 2: Click here to open the CIS form Step 1: Click on State Reports

  19. Click on your print icon or go to File-Print Box #1 is checked if history of chickenpox disease has been entered in the system

  20. Step 2: Click on All Recorded Vaccinations to capture ALL vaccinations Step 1: Click on Patient Record under Reports Step 3: Click on Create Report

  21. To Review… • Go to the website: www.waiis.wa.gov • Log in • Search for the student (patient) • View the student’s immunization record • Print the Certificate of Immunization (CIS) form • Give to parent/guardian to review, sign and submit to the school
