The aim of this training is to increase professionals awareness and understanding of CHC/FNC. • By the end of the session you will:- • Have a broad understanding of why and how the Continuing Healthcare (CHC) / Funded Nursing Care (FNC) Framework was developed, and why it is different from previous systems. • Have a detailed understanding of the overall process for determining CHC / FNC eligibility and the roles and responsibilities at each stage. • Understand the principles of good assessment and record keeping. • Have an understanding of the documents used in the CHC / FNC Framework process’
