  4. İliofemoral DVT Tedavisi – Circulation 2011, 123: 1788-1830. • İ.v. Bolus 80 U/kg standart heparin, i.v. Heparin infüzyonu 18 U/kg/s (5-7 gün) (Class I, Kanıt düğzeyi A). • DMAH : Enoksaparin 1 mg/kg günde 2 defa veya 1,5 mg/kg günde 1 defa, dalteparine 200 IU/kg tek doz, tinzaparine 175 U/kg tek doz (Class I, Kanıt düğzeyi A). • HIT saptanmış veya geçirmişse direkt trombin inhibitörü (Class I, Kanıt düzeyi B).

  5. İliofemoral DVT Gelişen Malignite Saptanmış Hastalar • DMAH ile en az 3-6 ay tedavi, kanser tedavisi devam ettiği sürece DMAH (Class I, Kanıt düzeyi A). • Çocuklarda DMAH kullanımı tartışmalı (Class IIb, Kanıt düzeyi C).

  6. Kateter Aracılığı ile Trombolitik Tedavi • Antikoagülasyona rağmen hızla ilerleyen trombüs varsa (Klas IIa, Kanıt düzeyi C) • Phelgmasia cerulea dolens varsa (Klas 1, Kanıt düzeyi B). • Sonuçları başarılı ancak geniş çalışmalara ihtiyaç var. • EKOS iyi bir alternatif olabilir

  7. Cerrahi Trombektomi • DMAH tedavisine cevap vermeyen • HIT gelişen • Trombolitik tedavi yapılamayan • Phlegmasia cerulea dolens ve alba dolens saptanmışsa (Klas I, Kanıt Düzeyi B)

İlginç Olgular • 20 yaşında 10 haftalık hamileliği olan bayan hasta • Sol alt ekstremitede DVT tanısı-Doppler USG • Tedaviye rağmen devam eden bel ağrısı • MR • Gormus N, Ustun ME, Paksoy Y et al.   11. İVK Obstrüksiyonlu Hastaların Bilgileri • 10 hastada IVK trombozu • 3 hastada gebelik • 4 hastada OKS kullanımı • Behçet, Abdominal Malignite, Protein C eksikliği • Yalnızca 3 hasta opere edildi!
