  2. Mission Mission Statement… • A person faced with a life-limiting illness deserves to live the rest of their life fully, comfortably and with dignity. United Hospice achieves this by providing dedicated quality holistic care, treating the physical, social and spiritual being as the patient approaches his or her final day.

  4. Pros Organizational Strengths… • 24 hour on-call services • Emotional and spiritual counseling • Inpatient services • Patient/family support and education • Services provided regardless of ability to pay

  5. Problem Central Problems… • Poor organizational structure • Out of date technology • Lack of communication tools

Solution to problem one New Organizational Structure • Administration is an oversight • Three teams consisting of one nurse and two CNAs • Each team will serve different patients depending on their location • Volunteers are not designated to specific teams because they are not a consistent resource. They will be applied wherever deemed necessary

  8. Solution to Problem Two Celltrak • Program that enables health professionals to enter patient specific care data into cell-traker interface • Pushes patient’s specific care plans and important information to staff upon arrival at the patient’s home (GPS monitoring allows this) • Easy to use

  12. Feasibility How this can work… • You will deduct $60 a month out of each employee paycheck in replacement of personal cell phone bill. • Will cover Verizon phone bill • Unlimited texting and data to receive emails and updates on patients • Total cost to United Hospice: $260 a month
