STRATEGY E: IMPROVE RETENTION IN CARE Activity E1: Expand provider education to improve capacity to retain clients. Activity E2: Implement pharmacy tickler programs Activity E3: Increase the number of Californians living with HIV who are enrolled in health insurance coverage. Activity E4: Improve integration of basic substance abuse/mental health interventions with HIV care settings. Activity E5: Strengthen relationships with organizations that provide housing support for clients in unstable or disadvantageous living situations
Surveillance Data • Local and state surveillance data have information on both Ryan White and other health systems and are the source for most data reports. The DATA-TO-CARE activities being implemented will also help achieve the Getting to Zero goals. • We are increasing the analysis of data of PLWH within the Medi-Cal program. • ARIES and LEO provides snapshots for services within your OA contracts. Cal-REDIE will be an additional resource.
Data is not Program • Data informs the activities you select for your integrated program. • Different activities will be selected based on your individual continuum and milestones reaching your IP Objectives. • Each LHJ will develop their integrated plan according to local needs, with input from providers and consumers.
