Activité en cancérologie • 95% des patients inclus sont cancéreux • Collaboration avec la plupart des établissements adhérents au réseau Onco 93 situés sur le territoire couvert mais aussi en dehors (Avicenne et Jean Verdier) • Dès que le projet n’est plus de guérir: patients parfois encore sous traitement spécifique à visée palliative Réseau Arc en Ciel www.reseauarcenciel.org

  3. Contenu de la collaboration • Organisation des retours à domicile et des réhospitalisations • Circulation de l’information entre la ville et hôpital • Participation au comité directeur d’Onco 93 Réseau Arc en Ciel www.reseauarcenciel.org

  4. Rôle propre Arc en Ciel • Coordination des acteurs du domicile • Rôle d’équipe mobile : expertise douleur et soins palliatifs • ponctuelle à un moment difficile de la maladie et/ou tout au long de la phase terminale • au domicile et dans les établissements privés Réseau Arc en Ciel www.reseauarcenciel.org

  5. Objectifs à développer ensemble • Communication interne et externe • Concertation autour du meilleur projet possible pour le malade • Décloisonnement ville - hôpital • Qualité des soins • Continuité de la prise en charge curatif/palliatif Réseau Arc en Ciel www.reseauarcenciel.org

  6. Propositions • Participation aux RCPO • Développement des soins de support avec implication des généralistes en ville avec l’existant dans les établissements privés en mutualisant nos moyens • Dossier médical partagé • Protocoles de soins • Formations • Médiatisation (colloque, écrit) Réseau Arc en Ciel www.reseauarcenciel.org
