How to Complete the Driver's Permit Application Form

This guide provides step-by-step instructions on how to fill out the driver's permit application. There are three key sections to complete: student information, parent signature, and applicant questions. Ensure that you use a black pen for all entries.   1. How to Fill out the Permit Application • There are 3 important sections to fill out on the permit application. #1 Student information and identifiers. #2 Parent Signature #3 Applicant Questions and Signature Please fill out the Application using only a black pen.

  4. Student Information • Please fill in your child’s name and information in the space provided. In all of the other boxes, please fill them in as they appear on this form.

  5. Parent Signature • Parents and/or Guardians please sign in the provided space and also fill in what your relationship is to the student. • Students or parents can fill in the remaining blanks with their information.

  6. Student questions You will find the student questions portion on the reverse side of the permit application. Students will need to only answer questions 1 thru 7 with a yes or no. Students will then need a signature in the provided space at the bottom right side of this page.

  7. Fees for Driver Education • Students will need to send $20.00 with the permit application. Cash and Checks are accepted. Please make any permit application checks payable to “Secretary of State”. Summer 2012 Lab Fees. • $250.00 • Payable to “Oswego High School” • Payments made in the Main Office. 2012/2013 school year Lab Fees. • $300.00 • Payable to “Oswego High School” • Payments made in the Main Office.
