Requirements • ALL cadets and prospective cadets must complete • Cannot compete for enrollment allocation or scholarship without a complete 48 on file • Complete the form online and print • Located at www.uwf.edu/afrotc under Cadet Area, under Forms, AFROTC Form 48 • You will need to download the Form Viewer to open • Or you can access from any computer in the Det lounge • AFROTC Classes • Fall Spring • AFR1101/AFR1101(L) AFR1112/AFR1112(L) • AFR2130/AFR2130(L) AFR2132/AFR2132(L) • AFR3221/AFR3221(L) AFR3232/AFR3232(L) • AFR4211/AFR4211(L) AFR4214/AFR4214(L) Academic class for AS100/200 year are 1 credit hour. AS300/400 year are 3 credit hours. Labs are all 0 credit hours.

Completion of 48 Type ALL information School/Det # Only one Major Self Explanatory Month and Year of DOG (include FY) BS, BA, etc., as applicable Academic Advisor signs/dates upon initial completion/review Cadet’s signature upon initial completion AFROTC instructor signature/date upon initial completion

  4. Completion of 48 e.g. Spring e.g. 2012 Enter credit hours course is worth; Place sum at bottom Indicated in university catalog To be completed during term review Use short, abbreviated course titles when possible

Requirements • Form should be completed online, printed and taken to your academic advisor for review • Academic Advisor Signature must be in blue or black ink • Include all semesters begin with your first semester in AFROTC and continue with Fall, Spring and Summer until Graduation • Complete a new Form 48: • Change in Academic Major • Change in institutions • Change in DOC/DOG going into new fiscal year

  7. Checklist • Did you accomplish all portions of the Form 48 online? • Are you taking 12 or more credit hours every Fall & Spring semester • Have you included all Summer semesters? • You must include Summer semester on the form, even if you are not taking classes • Have you printed the form and taken to your academic advisor for review? • Did he/she sign Block 4 in blue or black ink? • If you have answered YES to all questions see your academic instructor for review!
