  2. Telecare • Reduces the risks of daily living for vulnerable people • Supports everyone’s duty of care by delivering improved monitoring for alerts • Is something we are really keen to encourage professionals across Surrey to consider referring for, when appropriate

  3. Telecare gives effective alerts for: Falls Flood Smoke Extreme temp/fire Medication adherence …supports dementia sufferers with Fits Enuresis Geofencing Natural gas sensor CO sensor Door sensor Chair sensor

Telecare in Surrey • Mainstreamed 1/04/12 • Delivered by Boroughs & Districts • Common approach/charging across the county • lowest charge £22/month, highest £27 • equipment free for FACS Critical/Subs-tantial, service too if meet means tests • Supported by substantial awareness campaign running now

Telecare in Surrey (contd) • Telecare now available for 12 week free trial for any individual user likely to benefit • Aiming for 1500+ installs/year • 24 hr response service in train • Pilot soon • Adult Social Care is very keen to work with professionals across Surrey to encourage maximum referrals • Keen to establish win:win opportunities

  7. Importance of integrated care Could it be low blood pressure? Mr Taylor is beginning to fall Perhaps it’s due to trip hazards …or hypoglycemic comas Social Care Health …or ill fitting slippers …or a medication problem? What’s most important to Mr Taylor? …or poorly lit stairs? Integrated care

  8. Care Homes • Recently established a County-run project to encourage use of telecare in care homes • Will also tie in with substantial interest from Surrey Community Health in the use of telehealth in care homes
