ICAR Date & Time Incident Started: 4/11/05 12:10:00 PM Incident Description: During inspection of K-2308 H2 make up compressor it was discovered that a bolt resting on the 4th stage suction piping had worn a hole in the pipe and caused the release of make up hydrogen. The compressor was taken off line the depressured.

  5. The screw and nut mounted a temp gauge to a stainless steel plate.

  8. It appears that the inner wall of the sch. 160 pipe may have just been penetrated and began leaking Hydrogen because of the rubbing action of the screw and nut

  14. Two mechanics from Dressor Rand happened to hear the leak when they were working the compressor and contacted our operators. The operators responded immediately and safely shut the compressor down
