During the summer, campers jump into action, participating in exciting sports programs, exploring nature, becoming involved in performing and creative arts, playing a part in group activities, and attending special events. Our campers have the opportunity to expand their horizons during the summer, embarking on new adventures, and having fun! Our exceptional program and dedicated staff create an environment that forges lifelong friendships among our campers. We create a summer experience that is unforgettable! It all starts with our outstanding camp counselors! A camp is only as good as its staff, and we are extremely proud of ours. Talent, creativity, love of children, leadership and devotion make our camp counselors the heart and soul of the program.   3. Themes Summer Camp Weekly Themes This summer your children will be the “stars” of Camp! Camp Zone is filled with exciting “movie themed” adventures which will be sure to make long lasting memories. Each week we will have in-house field trips, fun activities, special guests, and water days! Our qualified and knowledgeable staff will guide your “star” on a fantastic summer-long journey! Week 1: “Monster’s Inc.” Week 6: “Harry Potter” Week 2: “Yogi Bear” Week 7: “Tarzan” Week 3: “Madagascar” Week 8: “The Incredibles” Week 4: “Honey, I Shrunk the Kids” Week 9: “Rookie of the Year” Week 5: “The Muppet Movie” Week 10: “Star Wars”

  4. Registration Information Welcome to Hollywood! We promise an exciting summer worthy of an Oscar! Your child will have the opportunity to explore new places, engage in new experiences and of course develop friendships that will last a lifetime. Please fill out the registration and permission slip forms on the next few pages and return them to the front office as soon as possible. This will ensure your child a spot in our camp. Summer Camp Activity Fees are listed to the right. They are collected in one sum to cover all costs associated with all the fun things scheduled on the camp calendar (i.e. field trips, in-house guests, t-shirts, crafts, activities, etc.) Please note ALL ENROLLED CHILDREN FOR SUMMER MUST PAY ACTIVITY FEES. NO EXCEPTIONS! Camp Fees Toddlers – $50 Beginners – $70 Intermediates – $85 PreK/PreK2 – $150 School Age - $300 • All fees are due to ChesterbrookNO LATER than Friday, May 31st.

Payment Policy: A nonrefundable deposit of one week's camp fees is required at the time of enrollment to hold your reservation. The deposit will be applied to the campers' final week of camp. Permission Slip Forms Child’s Name______________________________ Sunscreen Permission I give permission for the teachers of Chesterbrook Academy to apply sunscreen to my child throughout the spring, summer and fall. My child uses _______________________ (type of sunscreen). I understand that I am responsible for supplying sunscreen for my child and labeling the bottle with my child’s name. I also give permission for my child to walk to local destinations as outlined in the camp calendar. I have included the activity fee appropriate for my child with this permission slip. Movie Screening Forms School Age Campers ONLY Throughout the summer, there may be times (for example, a rainy day) where the School Age Campers may be watching the movies noted in the weekly themes (see movie titles below). Please note that some movies are rated PG. I give permission for my son/daughter__________________________ to watch the following movies (marked with an “X” below) at Chesterbrook Academy during Camp Zone Hollywood, 2013. I understand that if they are not allowed to watch said movie, they may be transferred to another classroom during the movie screening: • Monster’s Inc. Please note: campers are in groups of five or less, and are always with a counselor. They will be grouped according to ability levels, so please check only one selection. • My child needs to stay in the shallow end of the pool (water level no higher than mid-abdomen) and may use Splash Playground. • My child may go in water up to his/her chin level and may use Splash Playground and the Lazy River. • My child may go in water up to his/her chin level and may use Splash Playground, the Lazy River, and both Short Slides. • Special notes for counselors: ______________________________________________________________________________________________________________________ •  ______________________________________________________________________________________________________________________ • No floaties, flippers, kickboards or other accessories will be allowed at the pool. Children may bring goggles, earplugs, and life vests, if needed. • On swim days, children should come to camp with their swim suit underneath their clothes. They should have a backpack or a small duffel bag packed with a bottle of water, water shoes (not required), and a clean towel. Everything should be clearly labeled with your child’s name. Please apply sunscreen in the morning at home. Children will be changed back into their street clothes upon return to CA.
