  2. A traffic accident between a pickup and an 8,200 gallon gasoline tanker left two fatalities and the bridge beyond repair

  3. Friday, JANUARY 21, 2011 • 3:30 PM accident occurred. • 6:30 PM start setting up MOT to allow traffic past the site via exit and entrance ramps closing SR-3 • 8:30 PM DOT was allowed access to inspect the structure. • 10:00 PM DOT determines that 2 out of 3 spans on each bridge need replacement. • Contractors were called to attend a pre-bid conference at noon on Saturday 1/22/2011 for the replacement of two spans and one pier on both bridges and all repair associated with accident. • 10:30 PM start developing contract.

  4. Saturday, January 22, 2011 • 7:00AM to 11:00 AM Scope of work and MOT plan are complete • 12:00 PM pre- bid conference on the bridge site with Leware Construction, Johnson Brothers and Lane Construction. • 2:00 PM to 10: 00 PM receive and answer contractors bid questions.

  5. Sunday, January 24, 2011 • 8:00 AM DOT opened bids. Lane apparent low bidder at $2,191,000.00 and 25 days. • 8:30 AM contact awarded to Lane Construction. • 25 Days to have bridge Open with three additional weeks for cleanup. • $50,000 per day user cost penalty if traffic not re-opened by the 25th day, February 16th. • Sunday PM Lane starts mobilizing.

  6. Traffic Control A detour was set up to send Beachline traffic across the closed SR-3. To increase capacity the ramps were widened with temporary pavement.

  7. Contract Day 1 January 23, 2011 Mobilization and Demolition begins

  8. Monday January 24 Day 2 9:00 AM 4:00 PM Demolition continues

  9. Tuesday January 25 Day 3 9:00 AM 6:00PM Demolition Complete

  10. January 26 Day 4 Removal of Debris and slope pavement

  11. January 27 Day 5 Connection to existing footer Removal of fuel contaminated soil

  12. January 28 Day 6 Column Forms Pre tied cap steel

  13. January 29 Day 7 Columns and Caps poured monolithically

  14. January 30 Day 8 Final steel adjustment and prep for pour Concrete pour for piers

  15. January 31 Day 9 Clean up of existing structure Bearing pads and slope pavement

  16. February 1 Day 10 Beam setting

  17. February 2 Day 11 Beams set and columns stripped

  18. February 3 Day 12 Forming diaphragms and installing safety lines

  19. February 4 Day 13 Overhangs and deck forms being placed Barrier wall footing placement.

  20. February 5 Day 14 Stay in place forms and reinforcing steel being placed.

  21. February 6 Day 15 Continuing deck reinforcement Bidwell setup and steel adjustments

  22. February 7 Day 16 Reinforcing steel in place, Bidwell dry runs and clearance checks.

  23. February 8 Day 17 Eastbound complete 3:30 Westbound starting Start deck pour 9:00 AM Completion 6:30 PM, flying out the Bidwell

  24. February 9 Day 18 Removing edge forms and tying barrier wall steel Decks in place and curing

  25. February 10 - 14 Day 19 - 23 • Project work continued on Day 19 through opening on February 14, Day 23 with bridge rails, deck grooving, milling and resurfacing of roadway approaches damaged by fuel spill and fire, and final striping of the bridge and roadway.
