  2. Introduction • Project e-Pragati is about data collection, consolidation and generation of different reports related to Anganwadi centres and the beneficiaries associated with it for decision making purpose. • To meet the above objective data shall be collected in two phases • In Phase I, the data related to Infrastructure, Anganwadi worker and helper has already been collected. • The second stage of Project e-Pragati is to integrate the Anganwadi centers with the e-Shishu child details. Anganwadi Worker will also fill the formats for pregnant women and nursing mother.

  3. Objective • The main objective of e-Pragati Phase II is to collect the details of pregnant women, nursing mother and children. • This phase concentrates on collection of data for children between 0-6 year’s age. • In this phase the immunization and nutritional status of the child and pregnant women and nursing mother details are to be captured.

  4. Data Collection Process • The DSWO shall be the prime authority under whose supervision the entire data collection of a particular district shall take place. • Training shall be provided to at least two efficient personnel from each district we call them nodal officers. • These nodal officers shall then impart training to the CDPO/supervisors. These supervisors shall then provide training to the Anganwadi Workers on how to fill the survey formats. • Each nodal officers shall be provided the instruction manual and the survey formats.

  5. Data Collection Process • The child detail format provided for collecting the child details block wise/village wise shall contain the child name, child code, Guardians name, sex, caste & DOB. • The format shall be provided with 10 columns already filled rest 11 columns the Anganwadi worker has to fill. • For the child which is present in the Anganwadi register but is not available in the format, the Anganwadi Worker can add it as a new entry and can fill all the required information in all the 21 columns of the blank format..

  6. Data Collection Process • The child detail survey format provided to the Anganwadi Worker may also contain certain children information who is not a beneficiary of that particular Anganwadi Centre, such cases are to be left as it is, and Anganwadi Worker is not allowed to strike off any data. • Once the Anganwadi Worker completes the entry of all children available in the register then she would transfer the formats to the next Anganwadi Centre. • For capturing the pregnant women information, a blank format shall be provided where the Anganwadi Worker shall fill the information by referring the existing register.

  7. Roles & Responsibilities State Resource Coordinator • Two officers from State office will be responsible for the execution of the project e-Pragati in the entire state. • The district monitors will report to them on the status of the project on regular basis. • The state Resource Coordinator will provide all technical and other related information to the district and block level users whenever required.

  8. Roles & Responsibilities District Level Monitor: • The district monitor will monitor the entire process in his/her respective district. • The District level monitor will distribute the formats and master sheet to each block coordinator. Once the data is collected in the format, the block coordinator will deposit the same to the district monitor. • The district monitor will crosscheck the authenticity of the data by sampling method. If require s/he will visit a Anganwadi center and check the physical existence of the center. • The DSWOs or any other designated person will be the District level Monitors.

  9. Roles & Responsibilities Block coordinators. • The block coordinators are responsible for distributing and collecting formats from the supervisors. • This job can be taken up by the CDPOs at the block office. • A training session will be organized where the CDPO’s will be trained on the entire process by the district level monitors. • The block coordinators will collect the formats and instruction manuals from the district. • The Block co-coordinators will train the supervisors on the data collection process, when the formats are filled they will be submitted to the block coordinators who will cross check the formats and if require go to the field to cross check data in a sampling methodology.

  10. Roles & Responsibilities Supervisors: • The supervisors are responsible to provide adequate training to the Anganwadi Worker on filling the formats. • The supervisors would be responsible to monitor the filling of survey formats by the Anganwadi Workers. • The supervisors will receive training from the CDPOs who have attended training at the state level. • The supervisors will collect the formats from the CDPO at the block level and submit the formats after filling up with the details of Child and mother.

  11. Roles & Responsibilities Anganwadi Worker • Anganwadi Workers are responsible for collecting the child and pregnant women information of a particular village. • In e-Pragati phase II they will be responsible to fill different survey formats of child, Pregnant Women and Nursing Mother. • They will be provided adequate training by the supervisors.

  12. Point to be noted for Format fill up • For filling up the format English alphabets & numerical to be used. • The numbers to be written as 1,2,3,4,56,7,8,9……. • Pencil is to be used for filling the data. • Pencils should be sharp please doesn’t use blunt lids. This consumes more space and hence affects clarity. • AMIS Code is must for every data; any data with out AMIS code is liable to be rejected. • Date should be written in dd/mm/yyyyyy format any format for date other than this is liable to be rejected.

  13. Points to be noted for Format fill up • Weight should be written for e.g. 2.5 .Please don’t write 2.5 Kg or 2.500 Kg or 2500 gms or 2/500 • Each Pregnant woman/Nursing Woman format should contain only one AMIS code. More than one AMIS code in a single pregnant woman/Nursing Mother Format will be rejected. • AMIS Code should be correctly filled. It should not be more than or less than 8digits. • Avoid rubbing or overwriting a data, this creates ambiguity in data hence makes it becomes difficult for Data entry operator to read and enter. • Pregnant woman/Nursing mother Format to be filled first then only Child Format to be filled.

  14. Collecting Pregnant Women Details • The instruction manual provided to the Supervisor contains the formats to capture pregnant women details. • The format has 30 fields for capturing information • The formats provided are blank. • Anganwadi Workers are required to refer their register and fill different fields. • If they are not able to read/write in the language asked then they can take support from any other person of their area who can help them in this.

  15. Instruction to fill up format • The Anganwadi Worker is required to refer her registers before filling the survey format. • Below are the columns provided in the pregnant women survey format. AMIS CODE:- It is requested to the Anganwadi worker to put the AMIS code of her Anganwadi centre on top of the survey format. The AMIS code should be 8 digit numbers. If the Anganwadi Worker is not sure of the AMIS code she needs to consult her supervisor.

  16. Instruction to fill up format Col 1:- Sl No. in Register The serial number against each Pregnant woman/Nursing Mother should be same as the serial number of the Woman in the existing Register of the Anganwadi Centre. Col 2:- Name of Pregnant woman/Nursing mother Full name of the pregnant women/Nursing Mother has to be entered in capital letters. Col 3:- Husband/Father’s Name Full name of the husband/father of the pregnant women has to be entered in capital letters. Note:-Please mention title of the concerned person along with name. Col 4:- HH No. in Survey Register In this column the Anganwadi worker is required to enter the house hold number of the pregnant woman/Nursing mother, that she has captured in her survey register. Col 5:- Age Age of the pregnant women has to be written in two digits. Note:-Age has to be mentioned in figures e.g. 25,37,47 etc .Please donot write date like dd/mm/yyyy.

  17. Instruction to fill up format Col 6:- Caste The caste to which the pregnant woman belongs has to be provided in code. Please refer the codes below for the caste. Col 7:- BPL or APL The caste to which the pregnant woman belongs has to be provided in code. Please refer the codes below for the caste.

  18. Instruction to fill up format Col 8:- Regd Date The Anganwadi Worker has to refer her register and put the date on which the pregnant woman was registered. The date should be written in dd/mm/yyyy format. Note: -Please mention date of registration in dd/mm/yy format. If any format else other than date i.e. month or only year is mentioned it is liable to be rejected. Col 9:- LMP Date Here the Anganwadi Worker has to provide the Last Menstrual Date of pregnant woman. For this she can refer her register and can provide the date in dd/mm/yyyy format. Eg:- 15/11/2006 ,24/8/2007

  19. Instruction to fill up format Col 10:- Gravida Here the Anganwadi Worker has to provide the number of pregnancies a woman has had. The information has to be entered in digits. Note: - If a woman has given birth to twins then Gravida will be 1 If a child is born and dead then also gravida will be 1. Col 11:- Para Para indicates the number of viable birth .Here also the Anganwadi Worker has to provide the number in digits. Note:-If a woman gives birth to twins then Gravida will be 1 and Para 2 If twins and one dead then para is 1 If both alive then para is 2. Col 12. Still Birth Here the Anganwadi Worker has to provide the number of unviable births a woman has had, the information should be entered in digits. Col 13. Abortion Abortion indicates the number of pregnancy of a woman which went futile before birth. This information should be provided in numbers. Note: - In the format for Still Birth & Abortion it is written HISTORY that means here you are required to provide the past data.

  20. Instruction to fill up format Col 14:- Cholorquine If a woman has taken Cholorquine then the information is to be provided as 1 which indicated YES and if not then 0 which means NO. Col 15:- TT 1 Date Here the Anganwadi Worker has to provide the date (dd/mm/yyyy format) on which the pregnant women has taken her TT1. Col 16:- TT 2 Date Here the Anganwadi Worker has to provide the date (dd/mm/yyyy format) on which the pregnant women has taken her TT2. Col 17:- TT B Date Here the Anganwadi Worker has to provide the date (dd/mm/yyyy format) on which the pregnant women has taken her TTB.

  21. Instruction to fill up format Col 18, 19, 20: IFA A pregnant woman has to take 100 folic acid tablets in the slab of 30/30/40. Three columns provided for the same. If the pregnant woman has taken first 30 tablets then the Anganwadi Worker has to put () for IFA [I] i.e. column 18 Similarly if the pregnant woman has taken next 30 tablets then the Anganwadi Worker shall put a () for IFA [II]. i.e. column 19 If the pregnant woman has taken last 40 tablets then the Anganwadi Worker shall put a () for IFA [III] i.e. column 20. Col 21, 22, 23: Weight For weight the Anganwadi Worker will have to provide the weight during ANC1 if it is done. Similarly weight for ANC2 & ANC3 has to be provided if it is done, in their respective columns in the format. If no ANC done then put a X sign

  22. Instruction to fill up format Col 24:- Expected date of Delivery Referring her register the Anganwadi Worker can provide the expected date of delivery in dd/mm/yyyy format. Col 25:- Date of Delivery Date of delivery mentioned in the Anganwadi register has to be mentioned in the format in dd/mm/yyyy format. Col 26:- Place of Delivery If a pregnant woman given birth to a child in some hospital then code 1 to be mentioned for place of delivery ,similarly if a woman given birth at home under the supervision of trained Dhai then code 2 to be mentioned. If birth given at home under the supervision of others except Trained Dhai then code 3 to be mentioned.

  23. Instruction to fill up format Col 27:- Delivery Status Usually a pregnant woman gives birth to a live child but there can be cases a still child is born or a child got aborted before delivery or a pre mature birth takes place or an underweight child is born. All such delivery status has to be mentioned in codes. Please refer the codes below for the same. Col 28:-Covered under SNP If a pregnant woman is covered under SNP then the Anganwadi Worker has to mention 1 in the cell provided for the same & if she is not covered under SNP then 0 to be mentioned in the provided space.

  24. Instruction to fill up format Col 29:- PN Check up The number of Post Natal check ups a woman has taken has to be mentioned in numbers in the field. e.g. 1,2,3 Col 30:- REMARK A woman if dies after giving birth or left out of village or any thing else, has to be mentioned in the remarks column with provided code. Note:- If a woman comes under both pregnant woman & nursing mother category then two entries of a single woman has to be made.

  25. Collecting Child Details • The distribution of child formats (filled in and blank) will be distributed as in case of pregnant women/nursing mother formats. • In this format the, Child Name,Child Code, Guardian Name,Sex,Caste,DOB and above all the AMIS code are available, rest information the Anganwadi Worker can refer different registers and can fill the details. All the data shall be filled in CAPITAL letters. • The Anganwadi Worker shall collect the data from the registers to fill in the survey Format. • If the Anganwadi Worker unable to read/Write in the desired language then she can take help of any efficient person of the village and can get the survey format filled. • The format provided to an Anganwadi Worker may contain the names of Children who does not belong to that particular Anganwadi Center but is available in other centre. Please keep such child data as it is and don’t strike it off.

  26. Collecting Child Details • For the child whose information is not available in the format. the Anganwadi Worker is required to add all the information of such child as new entry. • Once the Anganwadi Worker fills the format, the same format shall be routed to next Anganwadi Center. In this way all the formats of a particular village shall rout through all the Anganwadi Centers located in that particular village. • All this exercise shall take place under the close supervision of Supervisors. • Once the format is filled the CDPOs shall verify the same. • The DSWO shall collect the formats from the block level supervisors and hand over to DPC of the particular district who will send it for data entry in the data entry zones.

  27. Instruction to fill up format There are 21 fields in the format. Col 1:- Sl No. in Register The serial number against each child should be same as the serial number of the child in the existing Register of the Anganwadi Centre. Col 2:-Child Name The child name shall be provided in capital letters. Col 3:-Child Code The child Code shall be same as e-Shishu, which will be provided in the survey format. Col 4:-Guardian Name as in e-Shishu This will be same as the e-Shishu database. Note:-If the guardian mentioned in the format or register is dead then it is recommended to change the guardian’s name to existing guardian’s name.

  28. Instruction to fill up format Col 5:-Relation with the Child The relationship of the child with the Guardian name mentioned has to be captured in this column. The codes for the same is as given below. Col 6:-HH No. in Survey Register In this column the Anganwadi worker is required to enter the house hold number of the child that she has captured in her survey register. Col 7:- DOB: - Date of Birth is already available. Note:-Please provide date in dd/mm/yyyy format. Date provided in dd.mm.yy format will be rejected.

  29. Instruction to fill up format Col 8:-Religion: S/he will correct the ‘R’ Religion of child as per the below code list if it is wrong in the format. Col 9:- Caste This data is already available in the format as it is present in e-Shishu Database.

  30. Instruction to fill up format Col 10:- Mother Tongue: S/he will correct the ‘M.T’ Mother Tongue of child if it is wrong in the format. If the mother tongue is 7- Other Tribal Language then the Anganwadi worker will collect & record the actual mother tongue in the following manner:

  31. Instruction to fill up format Col 11:-Gender This data is already available in the format as it is present in e-Shishu Database. Col 12:-Disability (If Any): S/he will correct the ‘Dis’ Disability Type of child if it is wrong in the format. Col 13:- Mother’ code The mother’s code for a child would be same as the Sl No of mother in the Mother’s Survey format.

  32. Instruction to fill up format Col 14 :- Birth Weight To provide the Birth weight the Anganwadi Worker has to refer her register and is required to put the same in the column of birth weight of respective child. Note:-Birth Weight has to be provided as 2.5 .Please donot write 2.500 or 2500gms 0r 2/500 etc .This is not only confusing but also consumes more space. Col 15:- 0 Dose Polio If a child has taken 0 Dose polio it has to be marked as 1 in the format otherwise 0 if not taken.

  33. Instruction to fill up format Col 16:- Immunization Status This particular column has been provided to know the nutritional status of the child. A code has been provided for each immunization. The Anganwadi Worker is required to refer the codes provided for each immunization before entering the information in the format. For example Case 1:- A child has taken only BCG, then she is required to put 1 in the immunization status column. Case 2:-A child has taken BCG, DPT I, Polio I, DPT II, then the Anganwadi Worker is required to put 1,2,3,4 in the immunization status column. Like wise she can put the respective codes for each immunization.

  34. Instruction to fill up format Col 17:- Number of referrals For this the Anganwadi Worker has to provide in digits the number of times a child has been referred. Col 18:- Grade as on Date Here the Anganwadi Worker has to provide the nutritional status of a child in code. For E.g.:-Case 1: If a child is normal then the code has to be 1 Note: - Grade has to be entered in figures provided in manual. Any thing else mentioned other than this i.e I/2, II/3 etc is not acceptable. Col 19:- Covered under SNP If a child is covered under SNP then the Anganwadi Worker has to mention Y in the cell provided for the same & if he/she is not covered under SNP then N to be mentioned in the provided space.

  35. Instruction to fill up format Col 20:- AMIS Code The Anganwadi Worker has to provide the AMIS code. Note:-The AMIS code should not be more than or less than 8 digits. Col 21:- Remarks A child if dies after birth or left out of village or any thing else has to be mentioned in the remarks column with provided code.

  36. Conclusion It is instructed to follow every instruction of the manual and fill the format accordingly. Any data found not adhering to instruction is liable to be rejected. The Anganwadi Worker in whose Anganawadi data the discrepancy is found will have to redo the entire data collection for her Anganwadi centre.
