Educational Outreach Collaborative Care Follow-up Small Group Learning Module on DCD Referral of Complex Children to Interdisciplinary Team (OT, PT, SLP, Psych) Small Group Discussion of DCD (DVD Movement Clips)[21] and/or OT Assessment in Office, Community or at Home (MABC,[22] K-BIT [23] & clinical interview) Feedback To Physician About Referral. Confirmation or rejection of DCD diagnosis Physician and OT Provide Information and Educational Materials to Family[31] Evidence-based Binder[29] Referral to School Health Services for Special Needs help and/or IEP and/or Referral of Child Evidence-based Website Access[21] and/or Review of “at risk” Cases No Referral: Family distributes materials to significant others (e.g. relatives, teachers, coaches).[31] Physician monitors long term and/or Parent Interview Guide[29] and/or Screening Tools[29] and/or Reflective Practice Chart Audit
