We have come a long way, but challenges remain Regulators focused on compliance rather than effective implementation Regulators are not looking at the intent of the regulation  Implementation has been very training centric And that training has focused on cost/compliance …and these organisations are not reaping the benefits and are becoming increasingly frustrated!

  11. Is this an organisation you know? HF? We've done that… We published our HF procedures We trained everyone in HF We comply with all current regulations. Our regulator audited us and it didn’t result in any findings regarding our HF programme We fix problems as they occur

What does a healthy HFEM programme look like? • What are the Key Performance Indicators? • A healthy reporting culture indicates: • Senior Management buy in • A real Just culture • Effective error investigation • Feedback system is working • Effective interventions • A reduction in error costs • If its monitored!

  13. THE MESSAGE ……its going to be a long journey …….….. but it is a shared journey We are heading in the right direction It’s about changing cultures and behaviours It’s about effectiveness not just compliance Our success is measurable
