Progress since Milano • The macrocost instrument presented at the Milano meeting was not found to be feasible • Two systematic reviews (2nd: 1,600+ hits, no relevant literature) • A third version of the macrocost-questionnaire was developed in close communication with UBI, ESKI/GYEMZSI and ISCII
