  3. Health Inspections • 3 most recent recertification surveys – recent surveys get higher weight • All complaint health deficiencies– last 3 years • Weighted score from number + Scope & Severity of deficiencies

  4. MDS 3.0 Quality Measures (available Summer 2012) • Long-stay Prevalence measures: • ADL change • Pressure ulcer prevalence • High and low risk • Long-term catheters • Physical restraints • Urinary Tract Infection ( UTIs) • Pain • Antipsychotic use (proposed) • Short-stay Prevalence Measures: • Pain • Pressure Ulcers • Short Stay Incidence • New antipsychotic use (proposed)

Staffing Data • Number of hours of care on average provided to each resident each day (HPRD) • Nursing Staff: RN, LPN/LVN & CNA • Case-Mix Adjusted – accounting for differences in the level of need for care of residents in different nursing homes (Case-Mix based on Resource Utilization Groupings (RUGS)

  6. Overall Rating • Rating System • Starts with health inspection results based on independent onsite visits; • Factors in Quality Measures and Staffing (both of which are self-reported)
